摘　　要：目的探索老龄社会来临后应对社区老年心血管病患者康复需求的有效措施。方法运用SWOT分析法,对老年心血管病患者回归社区后开展连续护理的内部优势和劣势以及外部环境存在的机会和威胁进行识别,并就老年心血管病患者的社区连续护理提出相关建议。结果对老年心血管病患者开展社区连续护理的优势明显,同时也受一定客观条件的制约;政策和专业发展为连续护理的实施创造了良好机遇,但是需要应对管理机制、服务模式、社会认可度等方面的挑战。结论社区开展连续护理需要结合实际,通过充分利用政策优势、加强人员培训、完善相关制度等,促使连续护理理念和实践在社区老年心血管病患者康复进程中发挥应有的作用。Objective To explore the effective measures for the rehabilitation demands among community elderly cardiovascular patients after the advent of the aging society. Methods Advantages and disadvantages of internal environmental, and the opportunities and threats of external environment of community continuing nursing in elderly cardiovascular patients were identified with SWOT analysis. Relevant suggestions were put forward about community continuing nursing of elderly cardiovascular patients with SWOT strategy matrix composite systems analysis. Results The continuous care for elderly cardiovascular patients had obvious advantages,and it was also affected by some objective conditions. Community continuing nursing had got good opportunity of implementation from policy and professional development, but it needed to deal with the challenges from mechanism, service model, social acceptance and other aspects. Conclusion The development of community continuing nursing needs to combine with the actual situation, make full use of the policy advantages, strengthen personnel training, and improve the relevant legal systems, so that the theory and practice of continuous nursing plays an important role in the rehabilitation process of elderly cardiovascular patients.
