不同剂型美托洛尔治疗慢性心力衰竭的临床疗效观察

摘　　要：目的比较不同剂型美托洛尔治疗慢性心力衰竭(CHF)的临床效果。方法将54例CHF患者随机分为常规组和缓释组,各27例。常规组在常规抗心力衰竭治疗的基础上加用酒石酸美托洛尔片;缓释组在常规抗心力衰竭治疗的基础上加用琥珀酸美托洛尔缓释片。观察两组治疗前后的左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)、脑钠肽(BNP),再次入院率及不良反应。结果两组治疗6个月后LVEDD、LVESD及LVEF比较,差异均无统计学意义(P>0.05);两组BNP水平比较,差异有统计学意义(P<0.05)。常规组总有效率为77.8%,缓释组总有效率为85.2%,两组疗效比较,差异无统计学意义(P>0.05);缓释组再次入院率为14.8%,常规组再次入院率为25.9%,两组比较差异有统计学意义(P<0.05)。结论美托洛尔能显著改善CHF患者的心功能,琥珀酸美托洛尔缓释片较酒石酸美托洛尔片可显著降低患者再次入院率及血浆BNP水平。Objective To compare the clinical effect of different dosage of metoprolol in the treatment of chronic heart failure （CHF）. Methods 54 cases of CHF patients were randomly divided into routine group and sustained -release group, with 27 cases in each. The routine group were treated with metoprolol tartrate tablets on the basis of routine anti heart failure treatment; sustained- release group with metoprolol succinate sustained -release＇ tablets in the treatment of heart failure on the basis of conventional. Observe two groups before and after treatment, the left ventricular end diastolic diameter （LVEDD）, left ventricular end systolic diameter （LVESD）, left ventricular ejection fraction （LVEF）, brain natriuretic peptide （BNP）, readmission rate and adverse reaction. Results The two groups after 6 months of treatment LVEDD, LVESD and LVEF comparison, the difference is not statistically significant （P 〉 0. 05 ） ; BNP level of two groups, the difference was statistically significant （P 〈0. 05）. The conventional group, the total effective rate was 77. 8%, sustained release group the total effective rate was 85.2%, compared with two groups of curative effect, there was no significant difference （P 〉 0. 05）; sustained release group again readmission rate was 14. 8%, the conventional group again readmission rate was 25.9%, there was significant difference between two groups （P 〈 0. 05）. Conclusion Metoprolol can significantly improve cardiac function in patients with C HF, metoprolol succinate sustained - release tablets can significantly reduce the patients in the readmission rate and the level of plasma BNP than metoprolol tartrate tablets.
