摘  要:目的探讨左肺切除联合野百合碱注射建立大鼠肺动脉高压模型的可行性。方法无特定病原体(SPF)级雄性sD大鼠22只,完全随机分为研究组(12只)和对照组(10只)。研究组大鼠切除左全肺,术后1周皮下注射野百合碱;对照组行假手术(仅单纯开胸),术后1周皮下注射0.9%氯化钠注射液。4周后比较2组大鼠平均肺动脉压(mPAP)、右心室肥大指数(RVHI)、肺小动脉管壁中膜厚度占血管外径百分比(WT%)、血管梗阻指数,并取右肺,比较2组病理学特征差异。结果研究组大鼠mPAP明显高于对照组[(42.4±2.8)mmHg(1mmHg=0.133kPa)比(19.0±2.6)mmHg],差异有统计学意义(P〈0.05);光镜下,肺小血管肌化程度增强,研究组RVHI、WT%、血管梗阻指数均明显大于对照组[(0.61±0.10)比(0.27±0.05)、(4.52-±1.19)比0、(26.38±4.27)比(1.82±0.13)],差异均有统计学意义(均P〈0.05)。结论左肺切除联合野百合碱注射用于建立大鼠肺动脉高压模型,模型大鼠肺血管结构发生明显变化。Objective To establish the model of pulmonary hypertenfion in rats with mon0crotaline(MCT) injection and left pneumonectomy to observe the pathologic changes. Methods Twenty-two spragne dawley (SD) male rats of specific pathogen free ( SPF ) grade were randomly divided into two groups : control group ( sham operated and sodium chloride injection; group B: 12 rats) and left pneumonectomy and M CT injection group( group A: 10 rats ). The mean pulmonary arterial pressure (mPAP), right ventricular hypertrophy index (RVHI) , the tunica media thickness percentage of small pulmonary arteries( WT% ) and index of blood vessel were examined in the rats after 4 weeks. The differences between the two groups with the fight lung and cardiac tissues were compared. Results The mPAP in group A increased significantly compared with that in group B [ ( 42.4±2.8 ) mm Hg vs ( 19.0 ± 2. 6 ) mm Hg, P 〈 0.05 ]. Pathological changes such as right ventricular hypertrophy were found in the rats 4 weeks after left pneumonectomy and MCT injection, significant pathological changes occurred including right ventricular fibrosis and thickening of the tuniea media of the pulmonary artery were also found. RVHI, WT% and index of blood vessel increased significantly [ ( 0.61 ±0. 10 ) vs ( 0.27 ± 0.05 ), ( 4. 52 ±0. 19 ) vs 0, ( 26. 38 ± 4. 27 ) vs ( 1.82 ± 0. 03 ) , P 〈 0. 05 ]. Conclusion Left pneumonectomy and MCT injection can induce the model of pulmonary hypertention, and there were significant pathologic differences of lung tissues between the two groups.
