手术治疗胰管结石患者的疗效分析

摘　　要：目的探讨胰管结石的诊治方法。方法回顾分析2002年1月至2011年12月16例胰管结石患者的临床资料，结合复习有关文献。结果16例患者14例伴有慢性胰腺炎、1例伴有胰腺癌者，3例伴有糖尿病。手术16例，其中胰管切开取石、胰管空肠吻合7例，胰十二指肠切除3例，单行胰体尾部切除术2例，胰体尾加脾切除术2例，保留十二指肠的胰头切除2例。结论胰管结石手术治疗方法应根据具体情况采取不同的手术方式，对胰头部胰管结石的患者可考虑行保留十二指肠的胰头切除；如行胰体尾切除治疗Ⅱ型胰管结石，尽量保留脾脏。Objective To discuss the diagnosis and treatment of pancreatoli thias. Methods The clinical data of 16 patients with Pancreatolithiasis treated in our hospital from 2002 to 2011 were retrospectively analyzed .Meanwhile , relevant literatures were reviewed. Results fourting patients were complicated with chronic pancreatitis. One had the complication of pancreatic carcinoma. 3 diabetes.All underwent operation. Pancreatic lithotomy plus pancreaticojejunostomy was performed in seven cases , p ancreatoduodenectomy in 4, distal pancreatectomy with spleen and the splenic vessels preservation in 2 and resection of the body and tail of pancreas plus splenectomy in 2. Duodenum-preserving pancreatic head resection （ DPPHR ） in 2. Conclusions The treatments should be given based on the situations of different patients. Duodenum-preserving pancreatic head resection （ DPPHR ） should be given to young patients with pancreatic duct stone of I type, and distal pancreatectomy with spleen and the splenic vessels preservation should be used instead of resection of the body and tail of pancreas plus splenectomy for patients with pancreatic duct stones of II type.
