湖北省荆门市某社区人群原发性高血压患者生活质量研究

摘　　要：目的评价湖北省荆门市东宝区宏图社区35岁及以上原发性高血压(EH)患者的生活质量并探讨其影响因素。方法使用社区人群功能状态测定量表(COOP/WONCA)中文版对符合条件的1 489人进行了调查。采用多因素二分类Logistic回归分析探讨影响人群生活质量的因素。结果原发性高血压患者在体能、社会活动、健康变化、整体健康、疼痛5个维度的得分高于血压正常者(均P<0.05);混合型高血压患者(systolic-diastolic hypertension,SDH)和单纯收缩期型高血压患者(isolated systolic hypertension,ISH)在体能和日常活动维度的生活质量较单纯舒张期型高血压患者(isolated diastolic hypertension,IDH)更低;罹患高血压对体能、整体健康和健康变化维度的生活质量有不良影响,年龄较大为EH的危险因素,常进行体力活动、男性、体力劳动者、收入水平较高为EH的保护因素。结论在今后的社区防治工作中须针对高血压病的不同类型和生活质量的可能影响因素,采取科学有效的干预措施。Objective To evaluate the quality of life （QOL） of patients with essential hypertension （EH） in Hongtu community,Jingmen and to explore the influencing factors of QOL. Methods The Chinese version of COOP/WONCA charts was utilized to measure the QOL of 1 489 subjects while multiple logistic regression analysis was used to explore the influencing factors. Results The scores of physical fitness, social activity, changes in health, overall health and pain were higher in patients with EH than in subjects with normotensives （P〈0.05）. Patients with systolic-diastolic hypertension （SDH） and isolated sys- tolic hypertension （ISH） had poorer QOL on physical fitness and daily activity than patients with isolated diastolic hypertension （IDH）. Hypertension had negative effects on physical fitness,overall health and changes in health. Old age was a risk factor for EH and physical activities, male, manual workers and higher income levels were protection factors. Conclusion According to different types of EH and the influencing factors of QOL,more scientific and effective programs should be adopted in the future.
