摘　　要：目的观察BiPAP无创通气在治疗重症肺结核并Ⅱ型呼吸衰竭中的临床疗效。方法对26例重症肺结核并Ⅱ型呼吸衰竭患者采用双水平气道正压通气(BiPAP)治疗,记录治疗前后血气分析的PH值、氧饱和度(SaO2)、氧分压(PaO2)、二氧化碳分压(PaCO2)等指标变化情况。结果治疗后患者的PaO2、SaO2升高、PaCO2降低,前后比较差异有统计学意义(P<0.05)。结论应用BiPAP无创通气治疗重症肺结核并Ⅱ型呼吸衰竭具有显著疗效,值得临床推广使用。Objective To explore the clinical efficacy of BiPAP non-invasive ventilation in the treatment of severe pulmonary tuberculosis and type Ⅱ respiratory failure. Methods 26 cases of severe tuberculosis and type Ⅱrespiratory failure patients were treated with bi-level positive airway pressure ventilation （BiPAP） therapy, observed blood gas analysis of PH,oxygen saturation （SaO2）,partial pressure of oxygen （PaO2）,carbon dioxide partial pressure （PaCO2） index changes. Results After treatment, PaO2, SaO2 of patients＇ increased,PaCO2 reduced, there were significant differences before and after treatment（P〈0.05）. Conclusion The efficacy of BiPAP noninvasive ventilation in the treatment of severe pulmonary tuberculosis and type Ⅱ respiratory failure is satisfactory, worthy of clinical promotion and application.
