摘 要:目的建立小鼠气囊植骨模型,观察纱布纤维是否会诱发炎性骨溶解,为优化手术操作奠定实验基础。方法选择2011年3—4月4例在我院行膝关节置换术患者丢弃的胫骨上端松质骨作为胫骨标本,均分为2块,1块采用干纱布擦拭(干纱布组),另1块采用0.9%氯化钠溶液浸湿的湿纱布擦拭(湿纱布组),通过电镜扫描并计算两组纱布纤维覆盖率。近交系SPF级BALB/c小鼠30只随机分为对照组、聚乙烯组和纱布组,每组10只,建立气囊植骨模型。肉眼观察小鼠处死前活动情况、死亡情况及处死后切口炎症反应情况;采用苏木素-伊红(HE)染色法观察小鼠囊壁组织单核细胞计数和淋巴细胞计数;抗酒石酸酸性磷酸酶(TRAP)染色法观察骨片破骨细胞阳性染色情况;免疫组织化学染色法观察囊壁组织白介素(IL)-1β、IL-6及肿瘤坏死因子α(TNF-α)表达水平,以平均光密度值表示;酶联免疫吸附试验(ELISA)检测囊壁组织TNF-α水平;实时定量聚合酶链反应(PCR)检测囊壁组织TNF-αmRNA表达水平,以TNF-αmRNA表达倍数表示。结果 (1)干纱布组纤维覆盖率为(18.11±6.94)%,湿纱布组为(4.86±3.64)%,差异有统计学意义(t=3.38,P<0.05)。(2)各组小鼠处死前活动情况良好,无死亡小鼠;处死后发现,聚乙烯组和纱布组小鼠切口存在不同程度的炎症反应,对照组小鼠无明显炎症反应。(3)各组小鼠囊壁组织炎症反应程度比较,差异有统计学意义(P<0.05)。(4)各组小鼠颅骨骨片TRAP染色阳性面积比较,差异有统计学意义(F=17.93,P<0.05)。(5)各组小鼠囊壁组织IL-1β、IL-6、TNF-α表达水平比较,差异均有统计学意义(F值分别为23.39、28.40和122.22,P<0.01)。(6)各组小鼠囊壁组织TNF-α水平及TNF-αmRNA表达倍数比较,差异均有统计学意义(F值分别为653.97和375.31,P<0.01)。结论纱布纤维与聚乙烯颗粒一样,可导致局部炎性细胞因子的释放增多而诱发炎性骨溶解。术中使用纱布�Objective To investigate whether gauze fibers can induce inflammatory osteolysis in mouse air pouch model in order to optimize operation procedures. Methods Cancellous bones from 4 patients underwent knee replacement in our hos- pital from March 2011 to April 2011 was divided into two parts, with one part wiped by dry gauze ( dry gauze group) and the other part wiped by wet gauze that was soaked by 0. 9% sodium chloride solution. The fiber rate of the gauze in the two groups was calculated by using SEM. 30 inbred SPF level BALB/e mice were randomly divided into control group, polyethylene group and gauze group with each group 10 mice. Air pouch model was then established. The activities before death, death and inflammatory reaction of wound after death were observed. The monocyte count and lymphocyte count in pouch walls were observed by using HE staining method. TRAP method was used to observe the positive staining of osteoclast. Immunohistochemical method was used to ob- serve the protein expression of IL - 1β, IL - 6 and TNF - α, which were expressed by mean optical density. Meanwhile, ELISA method was used to detect TNF -α and PCR method was used to detect TNF - α mRNA in pouch wall tissues, expressed by the expression times of TNF -α mRNA. Results ( 1 ) The residual level of gauze fiber in dry gauze group [ ( 18.11 ± 6. 94) % was significantly higher than that in the wet gauze group [ (4. 86 ± 3.64) %, t = 3.38, P 〈 0. 05 ] . (2) The mice activities in each group before death were normal. After death, the wound showed different levels of inflammation in polyethylene group and gauze group, while no obvious inflammation was found in the control group. (3) The inflammation reaction count in pouch wall tissues of each group showed statistically significant differences ( P 〈 0. 05 ) .
