3%硼酸加地塞米松湿敷联合莫匹罗星软膏治疗刺激性药物渗漏性损伤的临床研究

摘　　要：目的探讨3%硼酸+地塞米松湿敷联合莫匹罗星软膏治疗刺激性药物渗漏性损伤的临床疗效。方法选取2007年1月—2012年3月在我院住院期间发生刺激性药物渗漏性损伤的160例患者,随机分为试验组(80例)及对照组(80例),试验组采用3%硼酸+地塞米松液湿敷后外擦莫匹罗星软膏(百多邦),对照组采用50%硫酸镁湿敷,观察两组治疗前后渗漏组织的肿胀程度及疼痛程度,并于治疗1周后对两组患者的治疗效果及坏死发生率进行比较。结果两组治疗前肿胀程度及疼痛程度比较,差异均无统计学意义(Z=0.079、0.158,P>0.05);而两组治疗后比较,差异均有统计学意义(Z=1.502、2.293,P<0.05)。治疗1周后,试验组治愈72例,好转8例,无效0例;对照组治愈43例,好转25例,无效12例;两组临床疗效比较,差异有统计学意义(Z=2.29,P<0.01)。试验组未发生坏死,对照组发生坏死12例(15%)。结论 3%硼酸+地塞米松湿敷联合莫匹罗星软膏治疗刺激性药物渗漏性损伤效果显著,可有效消除组织肿胀,减轻疼痛,防治渗漏组织感染,促进损伤修复,防止坏死发生。Objective To investigate the effect of 3 % boric acid and dexamethasone wet dressing combined with Mupi- rocin ointment in the treatment of irritant drugs infusion extravasation injury. Methods 160 patients with irritant drugs infusion extravasation injuries hospitalized in our hospital from January 2007 to March 2012 were randomly divided into experiment group （80 cases） and control group （80 cases） . The experiment group was treated by 3 % boric acid and dexamethasone wet dressing combined with Mupirocin ointment, while the control group was treated by 50% magnesium sulfate wet dressing. The swelling and pain levels of extravasation tissues in the two groups were observed. After one week treatment, the treatment effect and necrosis rate was compared and analyzed between the two groups. Results The swelling and pain levels between the two groups before treatment showed no statistically significant differences （ Z = 0. 079, 0. 158 ; P 〉 0.05 ）, but showed statistically significant differences after treatment （ Z = 1. 502, 2. 293 ; P 〈 0. 05） . After one week treatment, in the experiment group, 72 cases were cured, 8 eases were improved and no case was invalid ; while in the control group, 25 cases were improved and 12 eases were in- valid. The treatment effect between the two groups showed statistically significant difference （ Z = 2. 29, P 〈 0. 01 ） . No case in the experiment group and 12 cases （ 15% ） in the control group had necrosis. Conclusion 3% boric acid and dexamethasone wet dressing combined with mupirocin ointment have good effect in the treatment of stimulant drugs infusion extravasation injury by al- leviating swelling and pain, preventing infection and necrosis and promoting repairing.
