肥胖小儿关节处静脉穿刺方法的探讨

摘　　要：目的:探讨肥胖小儿关节处静脉的最佳穿刺方法。方法:对160例踝部静脉难以触摸到的肥胖患儿随机分为传统组和改进组各80例。传统组遵照基础护理学的常规穿刺方法;改进组通过对局部拍打,用手指感觉血管的弹性、粗细、走向判断血管,并在血管最清晰处用针尖保护套压一印痕做记号,采用45～60度进行快速穿刺。结果:改进组一次成功率明显高于传统组,疼痛反应改进组疼痛率明显低于传统组P<0.05。结论:改进组穿刺方法可提高肥胖小儿关节处血管一次成功率且疼痛轻,值得推广。Objective To investigate the obese pediatric joint optimal puncture vein. Methods 160 cases of ankle vein to touch to the obese chil- dren were randomly divided into the traditional group and improvement group with 80 cases in each group. The traditional group in accordance with the basic nursing routine puncture method; improved group through the local beat, with the fingers feel blood vessel elasticity, thickness, to judge the vessel, and in vascular most clearly with the needle tip protective sleeve press imprint mark, with 45 -60 degrees for quick puncture. Results the improvement group the one time success rate is significantly higher than that of traditional group P 〈 0.05; pain response improvement group pain rate significantly lower than those in the traditional group P 〈 0.05. Conclusion the improved puncture method can improve the obese pediatric group joint vascular one time success rate and reduce the pain reaction, and worth spreading clinically.
