摘 要:目的:通过研究外周血单个核细胞早期凋亡率、fas系统基因及其蛋白表达水平,考察外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)早期凋亡率、mFas、mFasL、sFas和sFasL分子与SLE患者疾病活动度之间的关系。方法:病例选自两所三甲医院住院SLE患者,以健康志愿者为对照。利用流式细胞仪,采用连接素V和碘化丙啶双染法检测研究对象的PBMCs早期凋亡率及PBMCs膜上mFas和mFasL的表达水平;以ELISA测定研究对象外周血sFas和sFasL分子的表达水平。结果:SLE患者的PBMCs早期凋亡率(13.29%±4.12%,n=76)高于正常对照组(4.66%±1.57%,n=69)(Z=9.657,P<0.01),活动期患者(SLEDAI≥10)的早期凋亡率(14.80%±5.09%,n=53)高于非活动期(SLEDAI<10)(9.82%±3.76%,n=23),高于正常对照(4.66%±1.57%,n=69)(F=120.28,P<0.01)。多重线性回归分析结果表明,对于SLE患者和非活动期SLE患者,最终进入方程的自变量仅为PBMCs早期凋亡率(apoptosis)。对于活动期SLE患者,最终进入方程的自变量有PBMCs早期凋亡率(apoptosis)、血清Fas(sFas)两项。结论:SLE患者的PBMCs早期凋亡率显著升高,且与SLE患者疾病活动性指数的关系最为密切,临床上可作为SLE患者疾病活动度的监测参考指标之一;而对于活动期SLE患者,血清Fas(sFas)也能较好的反映患者疾病活动度。Objective:This study aims to observe the association between the early apoptosis rate of peripheral blood mononuclear cells (PBMCs) , mFas, mFasL, sFas, sFasL and disease activity in SLE patients by determining the expression of Fas system gene and protein as well as the apoptosis rate of PBMCs, so as to understand the rela- tionship between abnormal apoptosis and SLE. Methods : Patients with SLE were selected from two tertiary A - level hospitals, and normal healthy volunteers were recruited as controls. Annexin V (AV) and propidium iodide (PI) were used to detect the early apoptosis rate of PBMCs, and flow cytometry (FCM) was applied to detect the mFas and mFasL expression on PBMCs. Serum sFas and sFasL were assessed by Sandwich - enzyme - linked immunosor- bent assay (ELISA). Results: Patients with SLE had a significantly higher early apoptosis rate of PBMCs ( 13.29% ±4.12%, n=76) compared to healthy eontrols(4. 66% ±1.57%, n=69) (Z =9.657, P〈0.01), and active SLE patients ( SLEDAI ≥ 10) ( 14.80% ±5.09%, n = 53) had a higher apoptosis rate compared to inactive pa- tients ( SLEDAI 〈 10) (9.82% ±3.76%, n = 23 ).
