摘  要:目的采用心脏收缩调节器(CCM)的方法,观察低、高两种强度绝对不应期电刺激(ARPES)对慢性心力衰竭兔心功能的影响及可能机制。方法采用升主动脉套扎法建立兔慢性心力衰竭模型,将造模成功的实验动物随机分为对照组、ARPES-L组和ARPES-H组。在X线透视下沿颈总静脉放入心内膜刺激电极至右室心尖处,对照组未行任何强度ARPES,ARPES-L组和ARPES-H组分别给予电压为舒张阈值2倍和10V的ARPES 12h。采用无创心脏超声检测心功能、双抗体夹心ABC-ELISA法检测脑钠肽(BNP),以及荧光定量聚合酶链式反应(PCR)技术检测心肌膜连接复合体Junctophilin-2(JPH-2)mRNA转录水平的差异。结果与对照组比较,ARPES-L组与ARPES-H组左室射血分数增加,血BNP水平降低,JPH-2 mRNA的RQ值增加(P均<0.05),ARPES-L组和ARPES-H组上述指标无差异。结论短时间ARPES可改善慢性心力衰竭兔的心功能,JPH-2mRNA转录水平增加可能是增强心脏收缩力的机制之一。Objective Cardiac contractility modulation (CCM) is applied to observe the effects of electric stimulations with differenl amplitude during absolute refractory period (ARPES) on cardiac function of rabbits with chronic heart failure. Method The animal models of rabbits with chronic heart failure were performed by ligating ascending aorta's root. The successful models were randomly enrolled into control group, ARPES-L group and ARPES-H group. The experimental animals were sent an endocardial electrode through jugular veins into the apex of the right ventricle under the X-ray, the control group didn't give any ARPES. ARPES-L and ARPES-H group were given different amplitude during ARPES. The stimulant voltage was 2 times of diastolic threshold in ARPES-L group and 10 V in ARPES-H group. Ultrasonic, the ABC- ELISA method and the real-time fluorescence-based reverse transcription polymerase chain reaction were applied to examine the cardiac function, plasma brain natriuretic peptide(BNP) 's level and the differences of the mRNA expression of Juncto- philin-2 (JPH-2). Results Left ventrieular ejection fraction in ARPES groups were higher than in the control group( P〈 O. 05 ) , but there were no differents in ARPES-L and ARPES-H group. The plasma levels of BNP were significantly lower in ARPES-L and ARPES-H group than those in control group(P〈0.05). The RQ value were higher in ARPES-L and in ARPES-H group than those in control group( P〈0.05 ) , but there were no differents in ARPES-L and ARPES-H group. Conclusions The short-term ARPES could improve the heart function of rabbits with chronic heart failure. It may be one of the possible mechanisms that increase myocardial contractility through increased the mRNA expression of JPH-2.
