针灸治疗失眠症的临床疗效研究

摘　　要：目的探讨以三焦针法为主治疗失眠症的临床疗效及作用机制。方法将197例原发性失眠患者采用随机数字表法分为观察组（针灸气街）118例、对照组（西药内服）79例，观察组取穴头气街部为主，常规针刺，捻转补泻手法，行热敏灸；对照组口服艾司唑仑1mg，睡前服用，1次／d。采用匹兹堡睡眠质量指数（PSQI）评分进行疗效评定．观察两组疗效及治疗前后PSQI评分变化。结果观察组治愈72例，显效21例，有效23例，无效2例；对照组治愈16例，显效28例，有效26例，无效9例，经Ridit分析差异有统计学意义（P〈0．01）。治疗后各因子评分及总分比较，观察组在睡眠质量[（0．86±0．71）分vs．（1．07±1．65）分）、日间功能（（1．19±0．85）分vs．（1．30±0．69）分]、总分[（5．61±2．66）分vs．（5．03±2．46）分]方面优于对照组，差异均有统计学意义（P〈0．05）。结论针灸治疗失眠症的疗效优于服用艾司唑仑，为失眠症的治疗提供了新的手段。Objective To investigate the clinical effect of Sanjiao Acupuncture to insomnia and its mechanism of action. Methods 197 cases of patients with primary insomnia were according to random number table method divided into observation group of 118 cases receiving conventional acupuncture and moxibustion as Qi street point, and control group of 79 cases receiving Estazolam Tablets, I mg/d. The Pittsburgh sleep quality index （PSQI） score were to assess the changes before and after the treatment. Results There were 72 patients who were cured, 21 who got remarkable effect, 23 who got effect, and 2 who not responded in the observation group ; while in the control group the figures were respectively 16, 28, 26 and 9. Ridit analysis re- vealed a statistically significant difference. After the treatment, the PSQI factional scores of sleep quality [ （0. 86 ± 0. 71 ） vs. （ 1.07 ± 1.65 ）], daily function [ （ 1.19 ± 0. 85） vs. （ 1.30 ± 0. 69）] and the PSQI total score [ （5.61 ± 2. 66） vs. （5.03 ± 2. 46） ] in the two groups showed that the results of the observation group were statistically significantly better than those of the control group （P 〈 0. 05） . Conclusion The therapeutic effect is better in the observation group than in control group. It pro- vides a new therapeutic means for insomnia.
