温阳强心方治疗慢性心力衰竭临床疗效及其对血管紧张素Ⅱ和醛固酮水平的影响

摘　　要：目的探讨温阳强心方治疗慢性心力衰竭(CHF)的临床疗效及其对肾素-血管紧张素-醛固酮系统(RAAS)的影响。方法 60例阳虚水泛型CHF患者随机分为治疗组、对照组,在治疗原发病的基础上分别给予温阳强心方、依那普利片治疗4周,对比2组的心功能疗效、心衰积分及证候疗效,并对血浆血管紧张素Ⅱ和醛固酮进行比较。结果心功能疗效及心衰积分2组间比较差异无统计学意义(P>0.05)。治疗组中医证候疗效优于对照组,组间比较差异有统计学意义(P<0.05)。2组患者血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)水平均明显下降(P<0.01),其下降幅度2组间比较差异无统计学意义(P>0.05)。结论温阳强心方可改善CHF患者心功能和心衰症状,降低AngⅡ、ALD水平,中医证候疗效优于依那普利片。Objective To observe the clinical efficacy of Wenyang Qiangxin decoction and its effect on RAAS of patiens with chronic heart failure （CHF）. Methods Sixty case of patients with CHF （Yang deficiency and liquid stagnation syndrome） were selected and randomly divided into treatment group and control group, and treated with Wenyang Qiangxin decoction and enalapril respectively for four weeks. The therapeutic effect on classification of cardiac function, Chinese medical syndromes, heart failure score, Ang II and AID in plasma were observed. Resluits There was no significant difference between the two groups on cardiac function and heart failure score （P 〉0.05）. Wenyang Qiangxin decoction was better than enalapril on Chinese medicalsyndromes, with significant difference （P〈0.05]. Significant decease of Ang Ⅱ and AID were found in the two groups （P〈0.01）, with no significant difference between the two groups （P 〉0.05）. Conclusion Wenyang Qiangxin decoction can improve the heart function and symptoms of CHF patients, decrease Ang II and AID. Its efficacy on Chinese medical syndromes is superior to enalapril.
