B超介入宫腔镜输卵管通液术在评价输卵管通畅度中的作用

摘　　要：目的探讨B超介入宫腔镜输卵管通液术在评价输卵管通畅度中的作用。方法选择2008年1月至2009年12月因不孕症就诊于武警后勤学院附属医院并行宫腹腔镜联合手术的患者800例,术前均行B超介入宫腔镜输卵管通液术,分析并比较其与金标准——宫腹腔镜联合手术在输卵管通畅度诊断方面的效能。结果 B超介入宫腔镜输卵管通液术与宫腹腔镜输卵管通液术金标准比较,两者符合率86.06%,宫腔镜输卵管通液术的灵敏度为91.84%,特异度为58.48%,假阳性率(误诊率)8.69%,假阴性率(漏诊率)5.25%。结论 B超介入宫腔镜输卵管通液术具有一定的诊断和治疗效能,可作为不孕症的初步筛查手段。Objective To evaluate the effect of hysteroseopic hydrotubation guided by ultrasound in evaluating the fal- lopian tubal pateucy. Methods Eight hundred cases with infertility who were given laparoscopic surgery at the Affiliated Hos- pital of Chinese People＇s Armed Forces Logistics University from January 2008 to December 2009 were selected. All cases were practiced hysteroseope hydrotubation guided by ultrasound before operation and the diagnostic value between ultrasound guided hysterosopic hydrotubation and laparoscopy guided hysteroscopic tubal catheterization which was the golden criteria of tubal pa- tency diagnosis were compared. Results The diagnosis coincidence rate of the ]aparoseopy guided hysteroscope tubal catheter- ization and hysteroscope hydrotubation guided by ultrasound was 86.06%, the sensitivity of hysteroscope hydrotubation was 91.84% , the specificity was 58.48 % , the false positive rate was 8.69% and the false negative rate（ rate of missed diagnosis） was 5.25%. Conclusion Ultrasound guided hysteroscopie hydrotubation has both diagnostic and theraputic function. It can be used as an initital method for screening the infertility.
