摘  要:目的探讨伤后早期乌司他丁联合血必净干预对海水浸泡腹腔开放伤兔血流动力学及血电解质的影响。方法36只新西兰雄性大白兔被随机分为正常组、对照组1(腹部开放伤+海水浸泡)、对照组2(腹部开放伤+海水浸泡+平衡液)、实验组1(腹部开放伤+海水浸泡+乌司他丁)、实验组2(腹部开放伤+海水浸泡+血必净)和实验组3(腹部开放伤+海水浸泡+乌司他丁+血必净)。于浸泡60 min打捞出水时,对照组2、实验组1、实验组2、实验组3分别给予平衡液、乌司他丁、血必净及乌司他丁+血必净静滴(血必净1.8 ml/kg,乌司他汀5万U/kg,液体总量26 ml/kg);分别于浸泡前、浸泡60 min打捞出水及用药60 min时,观察各组平均动脉压、中心静脉压、外周血钠、钾、氯离子浓度的变化。结果乌司他丁、血必净均能显著提升海水浸泡腹腔开放伤兔的平均动脉压、中心静脉压,降低外周血钠、钾、氯离子浓度;乌司他丁联合血必净的作用更为明显。结论伤后早期应用乌司他丁联合血必净能显著改善海水浸泡腹腔开放伤兔的血流动力学及血电解质紊乱。Objective To discuss the effects of intervention with ulinastatin (UTI) and Xuebijing (XBJ) on hemodynamic parameters and blood electrolyte of rabbits in the early stage of celiac open injury soaked in seawater. Methods Thirty six New Zealand male white rabbits were randomly divided into 6 groups i. e. normal group, control groupl ( celiac open injury plus seawater immersion), control group 2 (celiac open injury plus seawater immersion and balanced solution), experimental group 1 (celiac open injury plus seawater and UTI ) , experimental group 2 ( celiac open injury plus UTI and XBJ), and experimental group3 ( celiac open injury plus sea water UTI and XBJ). Those rabbits were taken out of the seawater 60 min after the immersion, and the control group 2, experimental group 1,2, and 3 received the intravenous drip respectively with balanced solution, UTI, XBJ, and UTI plus XBJ (1.8 ml/kg XBJ,50,000 U/kg UTI, and 26 ml/kg total volume of liquid). Respectively before the immersion,60 minutes after the immersion ,and 60 minutes after the medication, observation was made in the changes of mean arterial pressure (MAP) ,central venous pressure (CVP), the concentration of sodion, kalium ion, and ehloridion in the peripheral blood. Results Both UTI and XBJ could significantly increase the MAP and CVP and decrease the concentration of sodion, kalium ion, and chloridion in the peripheral blood in rabbits after the celiac open injury with seawater immersion. Furthermore, the effects of the combination with UTI and XBJ were more significant. Conclusion UTI combined with XBJ used in the early stage of celiac open injury with seawater immersion can significantly improve the hemodynamic parameters and blood electrolyte in rabbits after.
