摘  要:目的探讨口服维生素A(VitA)联合新生期接种卡介苗(BCG)对实验性哮喘大鼠外周血CD4+CD25+调节性T细胞(Treg)及肺组织匀浆核因子(NF-κB)、白介素(IL)-4的影响,观察哮喘发作的情况,比较各组Treg数量变化及肺组织匀浆NF-κB、IL-4水平变化,从而为口服VitA联合新生期接种BCG在支气管哮喘中的防治提供理论根据。方法将57只1周龄SD大鼠随机分为4组:A组(n=15):口服VitA+腹腔注射BCG;B组(n=15):仅腹腔注射BCG;C组(n=15):哮喘对照组;D组(n=12):正常对照组。A、B、C 3组均致敏激发制作成哮喘模型,于激发后7、28、35 d取血50μl,检测Treg,取血后立即处死大鼠,切取肺组织块匀浆后以ELISA法检测IL-4及NF-κB水平。结果①哮喘大鼠体内的Treg较正常大鼠明显下降,同时伴有肺组织匀浆IL-4及NF-κB水平的持续性升高,到激发后35 d各值不能恢复正常;②肺组织匀浆IL-4水平与NF-κB水平呈显著相关性。结论①哮喘大鼠体内存在T细胞免疫功能紊乱;NF-κB可能是通过增强Th2类细胞反应参与哮喘的发病;②新生期接种BCG有利于纠正Th1/Th2失衡,起到调节免疫紊乱的作用;③Vit A可增强BCG调节免疫,具有协同作用。Objective To detect the impact of blood CD4 ~ CD25 ~ Treg and lung nuclear factors kappa B and IL- 4 with oral VitA and neonatal BCG vaccination on mice asthma model, and to provide a theoretical basis of oral Vi- tA and neonatal BCG vaccination at the prevention and treatment of bronchial asthma. Methods 57 rats were divid- ed into A, B, C,D four groups, Group A for the VitA and BCG group, Group B for BCG group, Group C for Asth- ma groups, and Group D for comparison. Rats in group A, group B and group C were made to establish asthma mod- el. 7 days,28 days and 35 days after the stimulation, the rats were taken in two batches, and peripheral venous blood from the orbital venous plexus were 50 ixl, with flow eytometry to detect CD4 ~ CD25 ~ T-cell. And then the rats were exemted to have their lung organization taken to test the level of NF kappaB and IL-4 by the ELISA law. Results The number of CD4 ~ CD25 ~ Treg of asthma is lower than that of non-asthma group, with the lasting in- creased level of lung homogenate NF kappa B and IL-4 in asthma group until 35days. The level of lung NF kappa B has liner correlations with IL4 . Conclusion Pathogenesis of bronchial asthma have a certain relationship with the disorder of T-cell immunity. NF kappa B takes part in the pathogenesis of bronchial asthma through inhaneing Th2 inflammation. Neonatal BCG vaccination can rectify the imbalance of Thl/Th2. VitA can strengthen regulating immunity and has synergy with BCG vaccination.
