摘 要:目的探讨维生素C(VitC)并促红细胞生成素(EPO)对大鼠肾脏缺血再灌注损伤的保护作用及其可能的机制。方法健康雄性Wistar大鼠50只,随机分为假手术组(A组)、缺血再灌注组(B组)、VitC预处理组(C组)、EPO预处理组(D组)、VitC并EPO预处理组(E组),在肾脏缺血再灌注24h后处死大鼠,留取标本,24h内检测各组血清肌酐(Scr)、尿素氮(BUN)水平;ELISA法测定血清中血红素加氧酶-1(HO-1)和丙二醛(MDA)的水平;苏木精-伊红染色光镜下观察肾组织的病理变化;免疫组织化学分析法检测肾组织HO-1的表达。结果与A组比较,B、C、D、E组血清BUN、Scr水平明显升高(F=414.63、387.89,q=9.58~6 388.47,P<0.05);与B组相比,C、D、E组血清HO-1水平明显升高(F=315.73,q=51.39~169.43,P<0.05)、MDA水平明显下降(F=38.97,q=5.59~10.62,P<0.05),肾组织HO-1表达明显升高(F=2 040.00,q=40.48~91.09,P<0.001),其中E组变化最为显著;与A组比较,B组肾小管坏死程度计分最高,E组肾小管坏死程度计分最低。结论 VitC与EPO单独或联合应用均能够诱导HO-1表达增加,抑制氧化应激,尤其是两者联合应用时作用更显著,提示两者对大鼠肾脏缺血再灌注损伤具有协同保护作用。Objective To explore the protection and its possible mechanism of combined vitamin C (Vit. C) and erythro- poietin on isehemia-reperfusion injury in rats. Methods Fifty healthy male Wistar rats were evenly randomized to five groups as follows: group A (sham-operation group) ; group B (ischemia-reperfusion group) ; group C (Vit. C pretreatment group) ; group D (Erythropoietin pretreatment group): group E (combined Vit. Cwith erythropoietin pretreatment group). The rats were sacrificed 24 hours after ischemia reperfusion, the specimens were kept. The levels of serum creatinine (Scr) and nitrogen (BUN) were de- tected within 24 hours' that of serum hemeoxygenase-1 (HO 1) and malonaldehyde (MDA) by EIJSA kit' after HE staining, the pathological changes of renal tissue were observed by light microscope' and the expression of HO-1 in renal tissue assayed by im munohistochemistry. Results Compared with group A, serum BUN and Scr levels of groups B, C, D, and E were much higher (F=414.63,387.89;q=9.58-6 388.47;P〈0.05) ; Compared with group B, the serum HO 1 in groups C, D, and E increased dramatically (F--315.73,q=51.39 169.43,P〈0.05), MDAlowered(F-38.97,q=5.59-10.62,P〈0. 05), theexpression of HO 1 in renal tissue elevated (F=2 040.00,q=40.48-91.09,P〈0. 001), the changes being most predominant in group E. The score of renal tubular necrosis was the highest in group B, and the lowest in group E versus that in group A. Conclusion Using Vit. C or EPO alone, or combination of the two drugs can increase the expression of HO-1, and inhibit oxidative stress, the role is more notable when combining Vit. C and EPO, indicating that both Vit. C and EPO have a synergistic protection on renal is- chemia-reperfusion injury in rats.
