摘　　要：从技术、人力资源、政策措施、体制与筹资问题等方面研究慢性病及老年人健康管理存在的问题,并提出改善技术、保障人力资源、完善政策措施、完善卫生体制与提高筹资水平的政策建议。研究结果表明,重庆市慢性病及老年人社区健康管理已取得了显著成效,但尚处于低水平阶段。Chronic disease and elderly health management issues existed in the technology, human resource, policy measures, systems and financing, etc. And suggestions from those above aspects were given to solve those issues, which included improving the technology, ensuring human resource, improving policy measures and health systems, and increasing financing. The research showed that the community health management for chronic disease and elderly had achieved effectiveness, but it was in a sub-optimal stage.
