摘 要:目的:分析2型糖尿病肾病大量蛋白尿患者肾活检病理特征及肾组织转化生长因子β1(TGF-β1)表达。方法:选择23例2型糖尿病肾病大量蛋白尿患者,行肾活检病理检查,检测计算足细胞密度,K-W结节的比例、系膜区增生程度、电镜观察肾小球基底膜厚度;免疫组化检测肾组织细胞TGF-β1阳性率。并与19例2型糖尿病肾病非大量蛋白尿组比较。结果:①DN大量蛋白尿患者足细胞密度较对照明显减少(P<0.01);②DN大量蛋白尿患者K-W结节、基底膜厚度、系膜区增生程度与对照组比较,差异无统计学意义(P>0.05);③DN大量蛋白尿患者肾组织细胞TGF-β1阳性率较对照明显升高(P<0.01)。结论:足细胞密度减少、肾组织细胞TGF-β1升高可能是糖尿病肾病大量蛋白尿的独立危险因素。Objective To analyze the renal biopsy characteristics of patients in type 2 diabetic nephropathy with massive proteinuria and renal tissue expression of transforming growth factor- β1(TGF-β1)Methods Select the 23 patients with type 2 diabetic nephropathy patients with massive proteinuria,Pathological examination of the renal biopsy, To calculate podocyte density,to detect proportion of K-W nodules,to detect the degree of proliferation of mesangial area,to detect the glomerular basement membrane thicknesswith electron microscopy.To detect the TGF- β1 positive rate of renal tissuewith Immunohistochemical detection.Compared with 19 cases with type 2 diabetic nephropathy of non-massive proteinuria.Results:(1)Podocyte density in patients of DN massive proteinuria significantly reduced compared with the control(P 〈 0.01);(2)K- W nodules,basement membrane thickness,the degree of mesangial area hyperplasia showed no significant difference compared with the control group(P 〉 0.05);(3)The positive rateof renal tissue of TGF- β1 was significantly higher compared with the control(P 〈 0.01).Conclusion The podocyte density reduced and renal tissue elevated TGF- β1 may be an independent risk factor for diabetic nephropathy massive proteinuria.
