个性化护理在癌症患者化疗期间PICC置管中的应用

摘　　要：目的讨论个性化护理在肿瘤患者化疗期间PICC置管中的应用效果。方法在哈尔滨医科大学附属第二医院肿瘤内科行化疗并PICC置管患者个性化护理质量管理中，选取41例患者作为研究对象，通过加强护士的理论知识与临床技能培训，对患者实行全方位的个性化护理质量管理，与55例患者对照分析，比较两组护士的理论与实践技能及PICC置管后患者的并发症情况。结果加强护士的个性化护理质量培训后，护士整体的理论和实践技能成绩明显上升，两组相比差异具有统计学意义（t值分别为3．237，2．434，2．674，2．635，2．360，3．120；P〈0．05），实验组患者PICC置管后并发症情况明显减少，降低了患者的非计划拔管率（2／41比6／55）和CRBSI（2／41比8／55）的发生率，两组相比差异具有统计学意义（χ^2值分别为3．891，4．258；P〈0．05）。结论在临床肿瘤化疗PICC置管患者实行个性化护理质量管理，能够显著降低患者的并发症，降低非计划拔管发生率，提高患者的临床疗效，值得临床推广借鉴应用。Objective To discuss the management of the quality of personalized care in cancer patients with PICC catheter during chemotherapy. Methods Totals of 41 patients who were in the hospital tumor chemotherapy with PICC catheter during chemotherapy received personalized quality management through strengthening nurse＇ s theoretical knowledge and clinical skills training as experimental group. And other 55 patients were received the routine nursing as control group, the complications of the patients were observed. Results The overall theoretical and practical skills scores of experimental group were increased significantly after strengthening the training of nurses in personalized quality of care, significant difference were found in the two groups （t = 3. 237,2. 434,2. 674,2. 635,2. 360,3. 120, respectively; P 〈 0. 05 ）, the complications number of patients with PICC catheter, the rate of patients with unplanned extubation （2/41 vs 6/55 ） and incidence of CRBSI （2/41 vs 8/55 ）, of experimental group were significantly lower than that of control group（ χ^2 = 3. 891, 4. 258 ,respectively;P 〈 0.05）. Conclusions Implementing personalized care quality management in clinical cancer chemotherapy in patients with PICC catheter can significantly reduce complications in patients and reduce the incidence of unplanned endotracheal extubation, and improve the clinical efficacy, so it is worthy of clinical oractice.
