摘 要:目的探讨乙型肝炎病毒(HBV)载量与ɑ-L-岩澡糖苷酶(AFU)、甲胎蛋白(AFP)的相关性。方法收集180例乙型肝炎病毒患者血清标本,检测其血清中HBV-DNA,根据其HBV-DNA载量不同进行分组;其中1组(HBV-DNA<5.0×102copy/ml)、2组(5.0×102copy/ml≤HBV-DNA<5.0×105copy/ml)、3组(HBV-DNA≥5.0×105copy/ml),每组60例,正常对照组60例;同时分别检测其AFU、AFP,对其结果进行分析;HBV-DNA检测采用荧光定量PCR法;AFU检测采用速率法,AFP的检测采用化学发光法。结果各载量组及对照组AFU之间比较差异无统计学意义(F=1.605,P>0.05);各载量组及对照组AFP之间比较差异有统计学意义(F=16.348,P<0.05);1组AFU与AFP之间有负相关(r=-0.286,P<0.05);2组AFU与AFP之间无相关性(r=0.219,P>0.05),3组AFU与AFP之间有正相关(r=0.774,P<0.05)。各乙型肝炎病毒载量组AFU阳性率之间无差异(χ2=4.192,P>0.05),各乙型肝炎病毒载量组AFP的阳性率随乙型肝炎病毒载量的上升有升高的趋势(χ2=6.863,P<0.05)。结论对乙肝患者,应同时检测乙型肝炎病毒载量及AFU、AFP,这样更能准备的观察肝脏的损害程度及预测肝炎病情的发展。Objective To explore the relationship between the serum concentration of hepatitis B virus DNA and the AFU, AFP. Methods Sera of 180 patients infected for HBV were collected. The serum samples were assigned into three groups( n = 60 in each group) based the different concentration of hepatitis B virus DNA: group 1 ( HBV-DNA 〈5.0 × 10^2copy/ml), group 2 (5.0 ×10^2eopy/ml ≤ HBV-DNA 〈5.0 × 10^5eopy/ml) ,group 3 (HBV-DNA≥5.0 × 10^5 copy/ml) and 60 normal serum sample as control group. The levels of AFU, AFP were determined and analyzed. Velocity method was used to detect AFU and chemiluminescence method for AFP Fluorescent quantitative PCR( FQ - PCR) was used to detect the HBV - DNA. Results The levels of AFU were not statistically significantly different ( F = 1. 605, P 〉 0. 05 ). The levels of AFP were statistically significant different ( F = 16. 348, P 〈 0. 05 ). There was negative correlation between AFU and AFP in group 1 ( P 〈 0. 05 ). There was no correlation between AFU and AFP in group 2 ( P 〉 0. 05 ), while positive correlation existed between AFU and AFP in group 3 ( P 〈 0. 05 ). The positive rate of AFU was not statistically significantly different ( χ2 = 4. 192, P 〉0. 05 ). Significant increase trend was related to positive rate of AFP as the increase of hepatitis B virus load. Conclusions The detection of the levels of AFU, AFP with the concentration of hepatitis B virus DNA at the same time is of very importance for observing the impairments of liver and the development of hepatitis.
