不同剂量右美托咪定对全麻患者尿量和电解质的影响

摘　　要：目的比较不同剂量的右美托咪定(dexmedetomidine,DEX)对全麻病人尿量和体内主要电解质的影响。方法择期颌面外科整形患者60例(ASAⅠ级),随机分为5组(n=12):生理盐水对照组(C组),DEX 0.2、0.5、0.8、1.1μg/kg 4组(D1～4组)。术中监测尿量和血浆中Na+、K+、Ca2+浓度,比较各时段数据差异。结果与C组相比,D1组尿量无明显变化(P>0.05),D2～4组明显增多(P<0.05),而D3与D4组相比无明显差异(P>0.05),且各组中上述离子浓度无明显变化(P>0.05)。结论 DEX具有利尿作用,并呈剂量相关性,大剂量(0.8μg/kg)具有封顶效应,且对Na+、K+、Ca2+浓度无明显影响。Objective To compare the effects of different doses of dexmedetomidine on urinary volume and electrolytes during general anesthesia. Methods Sixty ASA I patients,scheduled for oral and maxillofacial surgery were randomly assigned to 5 groups （ n = 12 each） ： control group （ group C ） ; dexmedetomidine 0.2,0.5,0.8, 1. lμg/kg groups （group D1-4）. The urinary volume and concentration of Na＋ ,K＋ ,Ca2＋ were recorded during the surgery at different time points. Results Compared with the urinary volume in group C, no significant difference （P 〉 0. 05） was found in group D1, significant difference（P 〈 0.05） was found in group D2-4. There was no sig- nificant difference between groups D3 and I24 （ P 〉 0.05 ）. There was no significant difference in concentration of Na ＋ ,K ＋ , Ca2 ＋ in each group （ P 〉 0.05 ）. Conclusion Dexmedetomidine has the effect on dieresis correlated with different doses. High dose of DEX has ceiling effect, but no effect on the concentration of Na ＋ , K ＋ ,Ca2＋ .
