摘  要:目的观察人β防御素-3(human beta-defensin-3,HBD-3)在人工关节感染患者松质骨中的表达,探讨其意义。方法按临床诊断收集术中切取的假体周围的松质骨和正常髂骨,并分为以下4组:人工关节感染组13例,假体无菌性松动组8例,占位器治疗后组9例,正常组7例。采用HE染色观察炎性细胞浸润,免疫荧光染色观察阳性细胞数目及强度并采用Image-pro plus(IPP)7.0C软件测量平均光密度值。收集术前外周血白细胞计数、红细胞沉降率(erythrocyte sedimentation rate,ESR)、C-反应蛋白(C-reactive pro-tein,CRP)结果。分析比较各组间差别。结果 HE染色4组无明显差别,免疫荧光染色以人工关节感染组阳性细胞最多、阳性程度及平均光密度值最高,依次为假体无细菌性松动组,占位器后组及正常组。术前外周血白细胞计数、ESR和CRP感染组最高,其余3组无明显差别。结论 HBD3在人工关节感染及假体无菌性松动患者的假体周围组织和松质骨中存在高表达,且在感染与松动间存在差异。Objective To study the expression of human beta defensin-3 ( human beta-defensin-3, HBD-3 ) in osteoporosis patients with prosthetic joint infections and to explore its significance. Methods According to the clinical diagnosis, cancellated bone around the prosthesis and normal iliac bone were collected and divided into the following 4 groups : artificial joint infection group ( 13 cases ) , aseptic prosthesis loosening group ( 8 cases ), spacer treatment group ( 9 cases ), normal group ( 7 cases ). HE staining was used to observe the infiltration of inflammatory cells, immunofluorescenee staining was applied to observe the positive cells number and intensity, and Image-pro plus ( IPP ) 7.0C software was also used to measure the value of the average optical density. Preoperative peripheral white blood cell count, erythrocyte sedimentation rate ( erythrocyte sedimentation rate, ESR ) , C - reactive protein (C-reactive protein, CRP) were collected and compared. Results There was no significant difference in HE staining among the four groups. Immunofluoreseence staining results indicated the most positive cells, highest positive degree and average optical density value in the artificial joint infection group ,followed by aseptic prosthesis loosening group,spacer treatment group and normal group. The highest preoperative peripheral white blood cell count, ESR and CRP were also observed in the artificial joint infection group, while no significant difference was found in the remaining three groups. Conclusion HBD-3 exhibits the highest expression in the peri-prosthetie tissue and cancellous bone in patients with artificial joint infection and aseptic prosthesis loosening. And there was also expression difference between artificial joint infection group and aseptic prosthesis loosening group.
