二维应变成像技术评价高血压并糖尿病病人左心室长轴功能

摘　　要：目的应用二维应变成像技术检测原发性高血压并2型糖尿病病人左心室长轴整体应变情况,并探讨其临床意义。方法选取健康志愿者30例(Ⅰ组)、原发性高血压病人35例(Ⅱ组)、原发性高血压并2型糖尿病病人35例(Ⅲ组),采集心尖四腔心切面、二腔心切面、左心室长轴切面的二维灰阶动态图像。应用二维应变成像技术测量左心室收缩期长轴整体应变率(GSrL S),舒张早期长轴整体应变率(GSrL E),房缩期长轴整体应变率(GSrL A)。结果Ⅱ组GSrL S和GSrL E低于Ⅰ组(F=15.58、17.11,q=3.39～4.48,P<0.01),GSrL A高于Ⅰ组(F=14.74,q=3.46,P<0.05)。Ⅲ组GSrL S和GSrL E低于Ⅰ、Ⅱ组(q=3.76～8.28,P<0.01),GSrL A高于Ⅰ、Ⅱ组(q=4.35～7.64,P<0.01)。结论二维应变成像技术可定量检测左心室心肌整体应变情况,准确评价原发性高血压并糖尿病病人左心室长轴功能的早期改变。Objective To evaluate the left ventricular long-axis （LVLA） function in patients with essential hypertension （EH） cornpiicating type 2 diabetes mellitus （type-2 DM） by using two dimensional strain imaging and investigate its clinical signifi- cance. Methods This study consisted of 30 healthy volunteers （group Ⅰ ）, 35 with EH （group Ⅱ ） and 35 with EH complica- ting type-2 DM （group Ⅲ ）. Two-dimensional grey-scale dynamic images were recorded from the apical four-chamber view, two- chamber view and LVLA. The global peak systolic longitudinal strain rate （GSrL S）, the global peak early diastolic longitudinal strain rate （GSrL E） and the global peak LV systolic longitudinal strain rate during atrial systole （GSrL A） were measured using two dimensional strain imaging. Results Compared with group I , the GSrL S and GSrL E in group Ⅱ decreased （F： 15.58, 17.11;q=3.39-4.48;P〈0.01） while GSrL A increased （F=14.74;q=3.46;P〈0.05）. Compared with groups I andH , GSrL S and GSrL E in group Ⅲ decreased （q=3.76-8.28,P〈0.01）, while GSrL A increased （q=4.35-7.64,P〈0.01）. Conclu- sion Two-dimension strain imaging technique can quantitatively detect the whole strain of myocardium of left ventricle, and accu- rately evaluate early changes of function of LVLA in patients with EH complicating type-2 DM.
