冠心病病人血清apoB与hs-CRP浓度变化及其意义

摘　　要：目的探讨冠心病(CHD)病人载脂蛋白B(apoB)和超敏C反应蛋白(hs-CRP)的变化及其临床意义。方法采用免疫透射比浊法和散射速率比浊法,测定59例CHD病人血清apoB和hs-CRP浓度变化,并与22例健康体检者(对照组)进行比较。结果 CHD病人血清apoB和hs-CRP浓度明显增高,与对照组比较差异有显著性(t=4.31,t′=9.17,P<0.01)。CHD病人血清apoB和hs-CRP的ROC曲线下面积分别为0.786和0.879。CHD病人各组间血清apoB和hs-CRP浓度比较差异有显著性,急性心肌梗死组>不稳定型心绞痛(UAP)组>稳定型心绞痛(SAP)组(F=11.73、17.47,q=3.155～8.359,P<0.05、0.01)。UAP病人血清apoB和hs-CRP浓度明显高于SAP病人(q=4.639、4.906,P<0.01)。结论血清apoB和hs-CRP浓度的变化对CHD具有一定的诊断价值,也可作为评价CHD危险程度的指标之一。Objective To explore the changes and clinical significance of serum apolipoprotein B （apoB） and hypersensi- tive C-reactive protein （hs-CRP） concentration in patients with coronary heart disease （CHD）. Methods The serum apoB and hs-CRP concentration were determin6d in 59 CHD patients by immunological transmission turbidimetry （ITA） and scatter velocity turbidimetry, respectively, and compared with 22 healthy subjects in the control group. Results The serum levels of apoB and hs-CRP in CHD patients were higher than that in the control group （t = 4.31,t= 9.17;P〈0.01）. The areas under the ROC curve of apoB and hs-CRP were 0. 786 and 0. 879 in the patients, the levels of serum apoB and hs-CRP in acute myocardial infarction （AMI） group were higher than that in unstable angina peetoris （UAP） group, and that in UAP group were higher than that in sta- ble anginapeetoris （SAP） group （F=11.73,17.47;q=3.155-8.359;P〈0,05, 0.01）. The levels of apoBand hs-CRPin pa- tients with UAP were obviously higher than that of patients with SAP （q=4. 639,4. 906;P〈0.01）. Conclusion The changes of apoB and hs-CRP concentration have a certain value in the diagnosis of CHD, which can also be used as one of the indexes to evaluate the severity of CHD.
