摘  要:目的探讨模拟深埋条件下大鼠挤压伤解除挤压后心电的动态变化规律。方法 Wistar大鼠36只,经钳夹法作用于双后肢[压力为(4.5±0.3)kg],置入常压低氧舱内,舱内氧浓度为10%±0.1%,并禁食水。持续72 h后解除挤压,恢复常氧环境,自由进食水。随机分为挤压解除后即刻、1 d、3 d、7 d、14 d、28 d 6个时间点,行体表心电图检查。所有大鼠均于挤压前后做自身对照。结果挤压解除后1只大鼠于14 h死亡。其余各时间点80%大鼠(28/35)心电图发生明显异常变化,其中挤压解除后即刻时间点全部发生,包括ST段下移、T波高尖、心律失常、异常Q波、QRS波群增宽及QT间期延长;1~3 d出现ST段下移、T波高尖、心律失常;7 d可见ST段下移、T波高尖;14~28 d仍见50%大鼠ST段下移。结论模拟深埋条件下挤压伤复合低氧缺水缺食(三缺)大鼠于72 h解除挤压后心电图可发生明显异常,提示心脏是该复合性挤压伤的敏感靶器官。Objective To study the changes on ECG after removal of crush in rats with crush injury in simulated deep buried condition. Methods Thirty-six Wistar rats were placed into an ordinary pressure hypoxic compartment with an oxygen concentration of 10%±0.1% without access to food or water after their hind limbs were clamped with a pressure of (4.5±0.3) kg. After the crush was consecutively removed for 72 hours, the rats were placed into an environment with a normal oxygen concentration with free access to food and water and underwent ECG on days 1, 3, 7, 14, and 28, respectively. Results Of the 35 rats with their crush removed, 1 rat died on 14 h, 28 had significant changes on their ECG, including ST-segment depression, tall peaked T-wave, QT prolongation, arrbythmias, abnormal Q wave on days 1-3, and ST-segment depression on days 14-28. Conclusion Significant changes can be found on ECG 72 hours after crush is removed in rats with crush injury in simulated deep buffed condition, indicating that heart is a sensitive target organ for combined crush injury.
