非梗阻性肥厚型心肌病左心房功能的初步研究

摘　　要：目的应用心脏磁共振对非梗阻性肥厚型心肌病(NOHCM)患者的左心房功能进行初步研究。方法纳入35例NOHCM患者(NOHCM组)和20名健康体检者(对照组)行心脏磁共振检查。根据左心房最大体积指数(LAVmaxI)是否增大,NOHCM组分为正常LAVI亚组和增大LAVI亚组。应用单因素方差分析比较对照组、正常LAVI亚组和增大LAVI亚组左心各功能参数的差异。结果与对照组比较,LAVI正常亚组左心房最小体积指数(LAVminI)增大,左心房体积变化分数降低(P<0.001),左心室体积缩小,左心室射血分数(LVEF)无显著性差异(P=0.055>0.05);而LAVI增大亚组LAVmaxI和LAVminI均增大,左心房体积变化分数明显下降(P<0.001),左心室体积和LVEF差异无显著性(P=0.731,P=0.535和P=0.521)。与正常LAVI亚组比较,增大LAVI亚组LAVmaxI和LAVminI均明显增大(P=0.002和P=0.017),左心房体积变化分数显著降低(P=0.002),同时左心室收缩末期体积指数增大,LVEF降低(P=0.033)。结论左心房功能可以评价NOHCM心功能的进展,其敏感性优于LVEF。Objective To preliminarily study the left atrium function in non-obstructive hypertrophic cardiomyopathy （NOHCM） by cardiac magnetic resonance （CMR）. Methods 35 patients with NOHCM and 20 healthy controls received CMR. NOHCM group was divided into two subgroups according to the left atrial volume index （ LAVI）, normal LAVI sub- group and enlarged LAVI subgroup. The differences of left atrial volume index in control group, normal LAVI and enlarged LAVI subgroups were calculated using one way variance analysis （ANVOA）. Results Compared with control group, LAVmlnI increased, LA fractional change decreased （P 〈 0. 001 ）, LVEDVI and LVESVI both decreased, LVEF had no significant difference （P = 0. 055 〉 0.05 ） in normal LAV subgroup; LAVmax I and LAVmin I both increased, LA fractional change decreased significantly （P 〈0.001 ）, as well as LVEDVI, LVESVI and LVEF had no significance （P =0.731 ,P = 0.535 and P = 0.521, respectively） in enlarged LAV subgroup. Compared with normal LAV subgroup, LA dilated （P = 0. 002 and P = 0. 017, respectively） and fractional change impaired significantly （P = 0. 002 ） in enlarged LAV group, while LV dilated and LVEF decreased （ P = 0.033 ） Conclusion It is feasible for LA function to evaluate cardiac function development in NOHCM, and its sensitivity is superior to LVEF.
