摘  要:目的评价电子血压计OMRON HEM-7012(示波法)对儿童青少年血压测量的准确性。方法根据美国医疗器械促进协会(AAMI)关于电子血压计准确性评价的要求,2011-02-08招募18岁以下儿童青少年,采集基本信息及身体测量指标。两名测量员采用多听筒听诊器使用台式水银血压计(听诊法)同时测量血压5次,电子血压计测量3次,二者间隔交替进行,记录每次测量血压值及脉搏。Bland-Altman法评价两法测量结果一致性,并分析与台式水银血压计比较,电子血压计测量之平均误差及标准偏差,按照AAMI标准进行评价。结果共完成有效测量110例,两测量员结果90%以上差值在5mmHg以内,100%在10mmHg以内。OMRON HEM-7012电子血压计与台式水银血压计测量收缩压差值95%介于-14.1~15.1mmHg,差值绝对值≤5,≤10和≤15mmHg所占比例分别为53.9%,83.9%和94.2%;舒张压差值95%介于-20.4~9.6mmHg,差值绝对值≤5,≤10和≤15mmHg所占比例分别为43.0%,73.0%和88.8%。OMRON HEM-7012电子血压计与台式水银血压计测量平均偏差为0.3/-4.7mmHg,小于AAMI标准5mmHg的上限;偏差标准差为6.8/7.0mmHg,<8mmHg的上限。结论 OMRONHEM-7012电子血压计平均误差及标准偏差均达到AAMI标准的要求,可用于儿童青少年血压的测量。Objective To evaluate the accuracy of the electronic sphygmomanometer OMRON HEM-7012 in blood pressure measurement among in children and adolescents. Methods According to the standard accuracy require- ments, defined by the Association for the Advancement of Medical Instrumentation (AAMI), for electronic sphyg- momanometers, children and adolescents under 18 years old were recruited. The anthropometric data was collect- ed. The right arm blood pressures were alternatively measured with multi-handset stethoscope mercury sphygmo- manometers for five times and with electronic device (OMRON HEM-7012) for three times by two measurers in a quiet room at proper room temperature. The measurement consistency of the two instruments was tested by Bland- Altman method. Compared with those of the mercury sphygmomanometers, the average error and standard devia- tion of the electronic device were analyzed and estimated by the standards of AAMI. Results A total of 110 valid measurements were completed. Over 90≤//oo of the differences in blood pressure readings got by two measurers were within 5 mm Hg, and 100% within 10 mm Hg. 95% of the difference values in systolic blood pressure (SBP) be- tween mercury sphygmomanometer and electronic sphygmomanometer were between --14.1 to 15.1 mm Hg. The percentages of absolute difference values in categories of ≤5, ≤10 and ≤15 mm Hg were 53.9%, 83.9% and 94.2% ,respectively. 95% of the difference values in diastolic blood pressure (DBP) were between --20.4 to 9.6 mm Hg. Its absolute difference values in categories≤5, ≤10 and ≤ 15 mm Hg were 43.0%, 73.0% and 88.8%, respectively. The mean variations in readings of OMRON HEM-7012 and mercurry sphygmomanometer were between 0.3/-4.7 ram Hg which were less than the maximum offsets of 5 mm Hg set by AAMI; the standard deviation of the offsets was 6. 8/7.0 mm Hg, less than the maximum offsets of 8 mm Hg.
