手术切除联合即时放射疗法治疗瘢痕疙瘩疗效分析

摘　　要：目的探讨手术切除联合即时放射疗法治疗瘢痕疙瘩的临床疗效。方法我们对82例瘢痕疙瘩患者均行手术切除联合24h内放射治疗，采用4～6MeV电子线进行照射，连续5d为一个疗程，单次剂量3—4Cy，总剂量范围15—20Gy。通过临床观察和定期随访记录治疗疗效及放射不良反应。结果82例患者中，治愈67例（81．7％），有效12例（14．6％），无效3例（3．7％）。总有效率（治愈＋有效）为96．3％，无效病例均为原病灶处复发，放射治疗随访期间未发现明显的不良反应。结论手术切除联合术后即时放射治疗是治疗瘢痕疙瘩的安全有效方法。Objective To investigate the clinical efficacy of surgical resection and postoperative radio- therapy immediately in keloid. Methods 82 patients with keloid were performed the surgical resection and postoperative radiotherapy immediately within 24 hours. The 4 ~ 6 MeV electron beam was adopted with a course of 5 days, the singl dose ranged from 3 Gy to 4 Gy each time and the totally dose ranged from 15 Gy to 20 Gy. Recording the therapeutic effect and radiotherapy toxicity reaction through clinical observation and regu- lar follow-up. Results Of 82 cases, 67 patients were cured （81.7%）, 12 patients were effective （ 14.6% ） and 3 patients were invalid （3.7%）. The rate of efficacy was 96.3%, which include cured and effective pa- tients. The invalid patients were all recurrent lesions in situ. No obvious toxicity reaction was observed by fol- lowing up. Conclusion Application of surgical resection and postoperative radiotherapy immediately in treating keloid is safe and effective.
