广州地区尖锐湿疣患者人乳头状瘤病毒的基因分型

摘　　要：目的分析广州地区尖锐湿疣(CA)患者人乳头状瘤病毒(HPV)的感染状况,为其临床治疗提供依据。方法提取228例CA标本的HPV-DNA,经聚合酶链反应(PCR)和反向导流杂交确定HPV型别,分析21种基因型的型别分布,评价HPV与CA的相关性;数据经SPSS16.0统计软件分析。结果 228例标本检出HPV阳性219例,总检出率为96.1%;219例CA患者中HPV低危型感染203例,占92.7%,HPV6型和(或)11型共189例,占86.3%;228例受检者共检出370株HPV,发现21种基因型别以HPV6、11、16、52、58型感染最常见;219例阳性标本中HPV单一感染120例占54.8%,多重感染99例占45.2%;219例HPV-DNA阳性的CA患者,低危型和高危型混合感染率为33.3%,不同年龄组混合感染率的差异无统计学意义(P>0.05)。结论 CA患者HPV感染以单一型别为主,HPV6和11型是主要型别,低危型和高危型混合感染较常见。OBJECTIVE To analyze the status of human papillomavirus（HPV） infections in the patients with condyloma acuminate（CA） in Guangzhou area so as to guide the clinical treatment.METHODS The HPV DNA were extracted from the specimens of 228 CA patients,PCR and reverse line blot hybridization were employed to determine the genotypes of HPV,the distribution of 21 genotypes was analyzed,the correlation between the HPV and CA was analyzed,and the statistical analysis was performed by using the SPSS16.0.RESULTS The overall prevalence of HPV in CA patients was 96.1%.Low-risk genotypes predominated,with the prevalence of 92.7%.The cumulative prevalence of HPV genotypes 6 and/or 11 was 86.3%.Totaly 370 strains of HPV were oletected in 228 participants.All 21 different genotypes were found,and HPV 6,11,16,52,and 58 were the most common genotypes.Of 219 positive samples,there were 120（54.8%） cases of patients with single HPV infection and 45.2% of the patients with multiple HPV infections.Of 219 CA patients with positive HPV DNA,the incidence of the mixed low-risk HPV infections and high-risk HPV infections was 33.3%,the difference in the mixed infection rate between various age groups was not statistically significant（P0.05）.CONCLUSION The single HPV infections is predominant in the CA patients,and the most prevalent genotypes of HPV are HPV 6 and 11.The mixed low-risk HPV infections and high-risk HPV infections are common.
