八年制医学见习生聚集性水痘疫情调查分析

摘　　要：目的调查分析发生在广州市某高等医学院八年制医学见习生一起聚集性水痘疫情,为临床医学见习生职业安全管理和医院感染防控提供参考。方法 2011年11月对医学见习生水痘疫情进行现场流行病学调查,密切接触者接受水痘-带状疱疹病毒(VZV)IgG抗体检测,并采取综合预防控制措施。结果共调查八年制医学见习生93名,5名医学生感染水痘,首发病例由带状疱疹引发,二代病例4名,2名同宿舍接触感染、2名同见习组接触感染,无三代病例;47名密切接触者中42名水痘抗体阳性,5名水痘抗体阴性医学生经21d隔离观察未见续发感染,总罹患率50.0%。结论医学院校及教学医院应加强医学生职业安全管理及水痘等传染病医院感染防控,早发现、早报告、早隔离是控制水痘疫情扩散的关键;为准确判断水痘易感者,应开展血清学检测。OBJECTIVE To investigatc a case of varicella ster epidemic among eight-year medical probationer of a medical college of Guangzhou so as to provide reference for prevention and control of nosocomial infections as well as for occupational safety management of the medical student during the probationer.METHODS We investigated the outbreak of varicella among medical students during clinical probationer in Nov 2011.All exposed medical students were tested for VZV immunoglobulin G（IgG）.Comprehensive measures of prevention and control were adopted to tackle the outbreak.RESULTS A total of 93 medical students were investigated and 5 medical students were infected by VZV.The first case was infected by varicella zoster.The 4 cases of second-generation consisted of 2 roommates of the first case and 2 classmates of the first case in the same trainee group.None of the third-generation case was found.42（89.4%） of 47 medical students were positive for VZV IgG.Five students without protective VZV-IgG had been isolated for observation for twenty-one days,and none of them developed an infection.The attack rate was 50.0%.CONCLUSION The medical school and clinic teaching hospital should work closely together to strengthen the management of occupational safety and nosocomial infection prevention for medical students.Early discovery,early reporting,and early isolation are the keys to control the outbreak of varicella.To accurately detect the patients who are prone to varicella,it is necessary to carry out the serology test.
