摘  要:目的探讨荧光定量PCR法(FQ-PCR)和微粒子酶免分析法(MEIA)检测血液传播性疾病的差异性,为医院开展医院感染监控提供参考。方法对2009年1月-2011年12月的578例住院受血患者采用荧光定量PCR检测法(观察组)和微粒子酶免分析法(对照组)分别检测人类免疫缺陷病毒(HIV)抗体、丙型肝炎病毒(HCV)抗体以及梅毒抗体,比较两种检测方法标志物阳性率的表达差异。结果观察组与对照组HIV抗体的阳性例数分别为6、4例,阳性表达率为1.04%、0.69%;HCV抗体阳性例数及表达率分别为7、5例,1.21%、0.86%;梅毒抗体分别为4、6例;0.69%、1.04%;观察组在HIV抗体、HCV抗体标志物阳性表达与对照组比较,检测精度高,差异有统计学意义(P<0.05),对照组对梅毒抗体的检测率高于观察组(P<0.05)。结论对行输血的患者进行输血前血液传染性疾病的检测,减少了医源性感染,两种检测方法配合应用,可以提高对传染病标志物的检测率,提高了判断受血患者健康状况。OBJECTIVE To evaluate the differences between the PCR assay and microparticle enzyme immunoassay(MEIA) in the detection of blood-borne disease so as to provide basis for the monitoring of nosocomial infections.METHODS A total of 578 cases of hospitalized patients from Jan 2009 to Dec 2011 were taken with fluorescent quantitative PCR method(theobservation group) and microparticle enzyme immunoassay method(the control group) to detect anti-HIV,anti-HCV and syphilis antibodies.The marker-positive rate of differentially expressed of the two methods was compared.RESULTS There were 6 cases positive for HIV antibody in the observation group,4 cases in the control group,the expression rates were 1.04% and 0.69%,respectively.There were 7(1.21%) cases of anti-HCV positive markers in the observation group and 5(0.86%) cases in the control group.There were 4(0.69%) cases positive for syphilis antibodies in the observation group and 6(1.04%) cases in the control group.The observation group was with higher accuracy than the control group in the detection of the anti-HIV markers and anti-HCV markers,the difference was statistically significant(P0.05).CONCLUSION The detection of pre-transfusion blood-communicable diseases for patients with blood transfusion reduces the iatrogenic infection.The combined application of two detection methods can improve the detection rate of infectious disease markers and improve the judgment of health status of the blood of patients.
