摘  要:目的通过蛋白芯片方法检测健康体检人群血清中幽门螺杆菌相关抗体的分布,评价蛋白芯片系统在幽门螺杆菌感染诊断中的临床应用价值。方法完成500名健康体检者的幽门螺杆菌蛋白芯片检测,并与诊断金标准13 C-尿素呼气试验(13 C-UBT)比较,评价其检测幽门螺杆菌感染的敏感性及特异性,描述幽门螺杆菌的毒力蛋白(CagA、VacA)抗体、癌变风险因子(Hsp60)抗体在幽门螺杆菌感染者中的分布特点及幽门螺杆菌对甲硝唑耐药性(RdxA)。结果与金标准比较,Ure抗体、CagA抗体、VacA抗体阳性的敏感性分别为85.1%、78.7%、44.7%,特异性分别为90.4%、92.0%、98.1%,准确度分别为88.4%、87.0%、78.0%;500名健康体检人群中,13 C-UBT检测幽门螺杆菌阳性率为37.6%,幽门螺杆菌感染中CagA抗体阳性率为78.7%,VacA抗体阳性率为44.7%,Hsp60抗体阳性率为40.4%,RdxA抗体阳性率为8.0%。结论幽门螺杆菌蛋白芯片抗体检测对诊断幽门螺杆菌感染具有较高的临床价值,可作为幽门螺杆菌现症感染的诊断方法。OBJECTIVE To study the distribution of related antibodies in Helicobacter pylori(H.pylori) from the serum of the population taking physical examination through protein chip so as to evaluate the clinical value of the protein chip in diagnosis of H.pylori infections.METHODS H.pylori protein chip detection was performed for 500 healthy population.13C-Urea breath test(13C-UBT)was the gold criterion of H.pylori infection.Serum samples of these population were obtained and the antibodies against cytotoxin associated gene A protein(CagA),vacuolating toxin A(VacA),urease,heat shock protein 60(Hsp60) and RdxA(nitroreductase) were detected by protein chip technique.RESULTS Compared with the gold criterion,the sensitivities of anti-Ure antibody,anti-CagA antibody,anti-VacA antibody in the diagnosis of H.pylori infection were 85.1%,78.7%,and 44.7%,respectively,the specificities were 90.4%,92.0%,and 98.1%,respectively,and the accuracies were 88.4%,87.0%,and 78.0%,respectively.Of 500 healthy population taking physical examination,the positive rate of the13C-UBT in diagnosis of H.pylori infection was 37.6%.The antibody detection showed the positive rates of anti-CagA antibody,anti-VacA antibody,anti-Hsp60 antibody and anti-RdxA antibody was 78.7%,44.7%,40.4%,and 8.0%,respectively.CONCLUSION The antibody detection by the protein chip is valuable in the diagnosis of H.pylori infection.
