摘 要:目的:观测周围神经损伤后应用原位神经移植与自体静脉小间隙套接修复方法的疗效比较,为临床治疗周围神经损伤选择合适的修复方法提供实验基础。方法:将45只Wistar大鼠的左右后肢共90侧随机分为3组,每组30侧。A组:将股神经切断3mm作为移植段,将移植段与神经远近两断端行原位外膜吻合。V组:将股神经切断3 mm,同时切取长约5~7 mm的同侧股静脉,将神经两断端送入游离的静脉两端,断端之间留一3mm的间隙,将神经外膜与静脉壁缝合。N组:对照组。术后16周,对股神经进行电生理及组织学检测。结果:再生神经纤维均可通过A组吻合口和V组的套接间隙,A、V两组神经传导速度及股四头肌张力与N组比较差异均无显著性(P〉0.05),组间差异也无显著性(P〉0.05)。结论:应用神经原位移植、自体静脉小间隙套接均可以修复周围神经损伤,且静脉小间隙套接修复可获得与神经原位移植相似的修复效果。Objective:To observe the difference in therapeutic effect of free nerve autograft with the small -gap by vein. To provide experimental foundation for clinical choice to treat peripheral nerve injury. Methods: 90 lower limbs of 45 Wistar rats were randomly divided into 3 groups. Group A: Cutting femoral nerve 3 mm as displantal section, which was anastomosed with neural stumps in situ. Group V: Cutting femoral nerve 3mm, and choosing 5--7 mm homolateral femoral vein at one time, maked two neural stumps into free femoral vein, keeping 3ram gap between two stumps, anastomosed epineurium with vein wall. Group N: control group. After 16 weeks, the femoral nerve were given electrophysiology-cal and histological examinations. Results:After 16 weeks, regenerative nerve fiber can get across the anastomotic stoma of group A and the gaps of groups V, nerve condutive velocity and the muscular tension of quadriceps femoris of group A and V had no significant difference with group N(P 〉0.05), had no significant difference between group A and group V. Conclusion: Nerve autograft in situ with the small - gap by vein gel canal could reparate peripheral nerve injury, but repairing effect of the small -gap by vein was analogical.
