经腹膜前间隙修补腹股沟疝腹腔镜路径与传统路径临床对比分析

摘　　要：目的:探讨腹腔镜完全腹膜外腹股沟疝修补术的安全性、微创性及美容效果。方法:比较分析腹腔镜完全腹膜外腹股沟疝修补术(128例)和传统开放腹膜外疝修补术(75例)两种术式的手术时间、术中失血量、切口美体评分、术后患侧术区疼痛、手术并发症等临床资料。结果:两组手术均获成功,无术后出血、切口感染等并发症。两者术后均有复发。单因素分析,腹腔镜组与传统组在年龄、性别、是否是单侧或双侧疝诊断、复发时间没有显著差异,在创面暴露时间、术后疼痛持续时间、术后疼痛程度上差异明显P<0.05。术中失血量、手术时间相比较,腹腔镜组明显少于传统组P<0.05。住院天数并没有明显差异。结论:腹腔镜完全腹膜外腹股沟疝修补术与传统开放腹膜外疝修补术相比,手术更安全、恢复快,可获理想的美容效果,是治疗腹股沟疝的有效可靠方法。Objective： Discusses the peritoneum/aparoscopic hernia repair outside of the safety of the opera tion, and minimally invasive cosmetic eft. Methods： More laparoscopic hernia repair the peritoneum completely technique （128 cases） and traditional open surgery outside the peritoneal hernia repair （75 cases） two of surgical operation time, perioperative blood loss, beautiful body score, postoperative incision of the operation area pain, surgical complications was clinical data. Consequence： Two groups are perfoed success, no postoperative compli cations, such as bleeding, infection of incision. Both have all the recurrence. Laparoseopie hernia repair complete ly outside the peritoneum group and the traditional open peritoneal oulside hernia repair group, single factor ar, aly sis, two groups in age, sex, is unilateral or bilateral hernia diagnos recurrence time no significant differences in the wound and postoperative pain exposure time duration, postoperative pain degree differences are significant P 〈 0.05. lntraoperatie blood loss, operation time, in comparison is less than that of traditional laparosgroup P 〈 0.05 Days in hospital and no significant differece. Conclusion ： Laparoscopic all the peritoneum groin hernia repair and the traditional way before the peritoneum gap compared the groin hernia repair, surgery safer, rapid recovery, and can obtain the ideal body effect, is the treatment of inguinal hernia operation method is effective and reliable.
