单枚cage置入结合RF-Ⅱ系统治疗峡部型腰椎滑脱症

摘　　要：目的探讨单枚cage椎间融合器后路斜向植入附加RF-Ⅱ椎弓根螺钉治疗峡部型腰椎滑脱症的临床疗效。方法 2007年7月～2010年12月间采用单枚cage椎间融合器附加RF-II型椎弓根螺钉内固定治疗47例Ⅰ～Ⅲ度峡部型腰椎滑脱,对本组病例进行回顾性分析。结果所有患者术后滑脱有不同程度复位,椎间隙增高,滑脱角改善;随访12～60个月,平均30个月,矫正程度无丢失,cage无下陷、滑移,椎间隙高度无丢失,所有病例术后12～18月均获骨性愈合。结论 cage椎间融合器能使滑脱椎体间接复位,直接撑开椎体前中柱,恢复椎间隙高度,维持腰椎的生理弧度,提高融合率;RF-II型椎弓根螺钉系统有直接复位滑脱椎体,两者结合能提供内在稳定和支撑作用,疗效满意,适用于峡部型腰椎滑脱症的治疗。Objective： To study the clinical efficacy of the posterior single interbody cage combined with pedicle of RF - Ⅱ vetebral arch screw fixation for treatment of lumbar spondylolisthesis. Methods： From July 2007 to December 2010, a retrospective study of 47 patients with Ⅰ ～ Ⅲ lumbar spondylolisthosis who underwent posterior single interbody cage combined with pediele of RF - Ⅱ vertehral arch screw intemalfixation. Results ： Reduction of different degree of spondylolisthesis, broadening intervertebral space and improving angle of spondylolisthesis were gotten in all cases after operation. With f1low - up from 12 ～ 60 months （ averge 30 months） the correcting angle, cage and screw position and height of interverbrd space were kept in this group. Bony unon. was obtained in all cases 12 ～ 18 months after operation. Conclusion： Interbody cage can indirectly correct listhetic vertebrae, Widen directly the anterior and middle columns of vertebrae, recover the height of the gap of intervertebrae, remain the physical radian and improve the rate of fusion. The system of RF - Ⅱ pediculns arcus vertebrae screw can directly correct the listhetic vertebrae. Combination of both can offer the internal stability and sustaining function. The curative effect is satisfactory. It is fit for treating the spondylolysis spondylolisthesis.
