摘　　要：目的　回顾分析 2 8例支气管哮喘病人的肺HRCT表现 ,并与胸部平片比较。方法　 2 8例临床诊断的支气管哮喘患者 ,年龄 2 0～ 75岁 ,平均 37 5岁 ,男性 9例 ,女性 19例 ,所有病人均有X线平片和CT扫描 ,HRCT采用1～ 1 5mm层厚 ,高频率重建算法和吸气末扫描 ,另 7例加扫呼气末HRCT ,肺窗条件窗宽 15 0 0Hu ,窗位 - 6 70～ -70 0Hu。记录X线平片及HRCT表现并对比分析。结果　X线平片见肺容积增加和肺透亮度增加 4例 (14 3% ) ,肺纹理增粗 5例 (17 9% ) ,纵隔气肿 1例 ,总阳性率 2 5 % (7/ 2 8)。HRCT表现呈支气管壁增厚 15例 (5 3 6 % ) ,支气管扩张 6例 (2 1 4% ) ,马赛克征象 3例 (11 8% ) ,纵隔气肿 1例 ,小叶中心型肺气肿 4例 ,呼气末空气潴留 5例 ,总阳性率 71 4% (2 0 / 2 8)。结论　HRCT比X线平片更有效显示支气管哮喘患者的肺内异常表现 。Purpose To describe high resolution computed tomography(HRCT)findings of asthma and compare those findings with chest plain films. Methods 28 asthmatic patients(9M,19F;age:20～75 years,M=37.5)who had undergone both HRCT of lungs and chest plain films were evaluated.HRCT was performed with a thin section(1～1.5 mm collimation)technique and a high freguency reconstruction algorithm.In all subjects,end inspiratory scans were obtained and 7 patients were also performed with end expiratory scans.The images were obtained at window levels of -670～-700 Hu and at a window width of 1500 Hu. Results On chest plain films,4 cases showed lung volume increasing and pulmonary parenchymal transparency increasing(14.3%),predominant lung marking in 5 cases,mediastinal emphysema in one case.The positivity of chest plain films was 25%.Of these 28 patients with HRCT scans showed bronchial wall thickening in 15 cases(53.6%),bronchiectasis in 6 cases (21.4%),mosaic pattern in 3 cases(11.8%),mediastinal emphysema in one case and central lobular emphysema in 4 cases,air trapping on end expiratory CT scans in 5 cases.The positivity of HRCT scans was 71.4%. Conclusion HRCT may be useful for evaluating pulmonary abnormality of patients with asthma and more valuable than chest plain films. [
