摘  要:目的:利用锥形束CT(cone-beam computed tomography,CBCT)扫描上颌第一磨牙根管系统,分析其根管形态,探讨其近颊根增龄性变化,指导临床根管治疗。方法:将门诊患者分为≤40岁和>40岁两组进行CBCT扫描,分别进行上颌第一磨牙近中颊根根管系统的观察分析。结果:①A组MB2发现率为80.4%,B组为67.2%,差异有统计学意义(P<0.05)。②A组近颊根近远中向弯曲度:28.5±8.2°,B组:26.2±7.7°,差异有统计学意义(P<0.05);A组颊舌向弯曲度:16.2±9.1°,B组:17.0±7.4°,差异无统计学意义(P>0.05);A、B组弯曲部位均绝大多数位于根中1/3。③MB-MB2间距A组:1.3±0.5mm,B组:1.5±0.5mm,差异有统计学意义(P<0.05)。④颊根长度,A组:17.9±1.4mm,B组:17.7±1.3mm,差异无统计学意义(P>0.05)。结论:上颌第一磨牙近颊根MB2发现率会随年龄增大而降低,特别在40岁之后会出现明显下降趋势;上颌第一磨牙近颊根根管复杂多变,且多数会出现明显向远中的弯曲;CBCT能辅助临床复杂根管的治疗。Objective: Using CBCT to investigate maxillary first molars root canal system, analyze the second canal of mesiobuccal root and the curvature of the root. Methods: CBCT scanning was performed on 153 Chinese patients which have been divided into two groups (≤40y, 〉40y). The CT images were analyzed to determine the incidence of the MB2 canal, the anatomical structure and the correlations between the incidence of MB2 canal and age. Results: (1) The incidence of MB2 canal in group A was 80.4%, significantly higher than group B 67.2% (P〈0.05). (2) The curvature in group A was 28.5± 8.2° in mesiodistal direction while group B was 26.2± 7.7°, with statistical differences (P〈0.05). The curvature of group A was 16.2± 9.1° in buccolingual direction while group B was 17.0± 7.4° without statistical difference (P〉0.05). It has been shown that curved regions in group A and B were often found out in the middle 1/3. (3) The average distance between the main canal orifice and the MB2 canal orifice was 1.3± 0.5mm in group A, while 1.5± 0.5mm in group B with significant differences (P 〈0.05). The buccal root was 17.9± 1.4mm in group A, while 17.7± 1.3mm in group B without statistical difference (P 〉0.05). Conclusion: The incidence of MB2 canal in mesiobuccal root of maxillary first molars was showed to decline along with the increase of age. The canal system in the mesiobuccal root of maxillary first molars was varied and complex, mostly with buccolingual curve obviously. CBCT is an outstanding assistant examination to the root canal therapy.
