摘  要:目的探讨自体组织工程骨软骨复合体修复关节软骨缺损的可行性,评价修复效果。方法体外培养扩增猪关节软骨细胞及成骨细胞,分别接种于聚乳酸聚羟基乙酸聚合物(PLGA)和磷酸三钙(TCP)上培养2周后,缝合成骨软骨复合体。复合体植入关节软骨缺损为实验A组,单纯工程软骨为对照B组,不做处理为空白c组。回植3个月后检测修复效果。结果A组缺损修复组织表面光滑平坦,与周围软骨整合良好。B组和C组的修复组织呈纤维组织和无修复。组织学显示A组结构致密,细胞外基质分布更均匀,与周围软骨及软骨下骨整合良好。A组GAG含量为(571.50±12.77)肛g,优于B组(410.67±17.36)μg、c组(308.17±19.34)μg,差异有统计学意义(P〈0.01)、A组Ⅱ型胶原染色面积为(90.07±2.45)%,优于B组(79.53±4.72)%、C组(66.17±2.94)%,差异有统计学意义(P〈0.01)。结论骨软骨复合体移植物能实现早期固定并有效修复关节软骨缺损。Objective To explore the feasibility of repair of articular cartilage defects with autolo- gous engineered osteochondral complex, and evaluate the efficacy. Methods Chondrocytes and osteoblasts were isolated from swine articulars cartilages and expanded respectively in vitro. Chondrocytes were seeded into a poly lactic-co-glycolic acid (PLGA) scaffold, while osteoblasts were seeded into a TCP scaffold. They were cultured for 2 weeks, and sutured into osteochondral complex. In group A, the osteochondral complex wa~ transplanted to repair articular cartilage defect. In group B, the engineered cartilages were transplanted, and in group C, nothing was given as the control group. They were harvested and examined after 3 months. Results Their surfaces of the tissues in group A were smooth and glossy. Fibrous tissues were found'in group B, and there were no changes in group C. The distributions of cartilaginous extracellu- lar matrices in group A were more homogenous than the others. The contents of GAG in group A were (571.50±12.77) μg, higher than in group B [(410.67±17.36)μg], and group C [(308. 17±19. 34 ) ixg] , and the differences were statistically significant (P 〈 0. 01 ). The percentages of collagen Ⅱ dyeing area in group A were (90. 07±2. 45)%, higher than in group B [ (79. 53 ±4. 72)% ], and group C [(66. 17 ±2.94)% ], and the differences were statistically significant (P〈0.01). Conclusion The osteochondral complex can fix early and repair the articular cartilage defect effectively.
