2型糖尿病患者焦虑情绪与糖脂代谢相关性研究

摘　　要：目的评价2型糖尿病患者焦虑情绪与糖脂代谢紊乱的相关性.方法初诊2型糖尿病患者89例,采用Zung焦虑自评量表（SAS）进行评分,依据评分结果分为2型糖尿病合并焦虑情绪组（DA）37例和2型糖尿病非焦虑情绪组（NDA）52例,对照组为同期健康体检者（NC）33例.记录受试者一般资料,测定空腹血糖（FPG）、糖负荷后2h血糖（2hPG）、糖化血红蛋白（HbA1c）、血脂和高敏C反应蛋白（hs-CRP）.结果与NDA组比较,DA组的心率、FPG、2hPG、HbA1c、TG、LDL-C及hs-CRP水平均显著升高（P均〈0.05）;相关分析显示,2型糖尿病患者SAS 评分与心率、FPG、2hPG、HbA1c、TG、LDL-C及hs-CRP水平呈正相关;多元逐步回归显示,2hPG、HbA1c、LDL-C及hs-CRP水平对2型糖尿病焦虑情绪具有显著性影响（P均〈0.05）.结论2型糖尿病焦虑情绪与糖脂代谢紊乱、慢性炎症反应密切相关,其中2hPG,HbA1c,LDL-C及hs-CRP可能参与了2型糖尿病患者焦虑情绪的发生.Objective To evaluate the correlation of glucolipid metabolic disorders and anxiety in type 2 diabetes. Methods Based on Zung self-rating anxiety scale（SAS） ,89 newly diagnosed type 2 diabetes patients were divided into two groups,37 cases accompanied with anxiety in group l （DA） and 52 cases without anxiety in group 2 （ NDA）. 33 healthy people were chosen as control group （ NC ） for the same period. Glyeosylated hemoglobin, fasting plasma glucose,2h plasma glucose, blood lipid analysis and hs-CRP were measured and general informations were recof ded. Results Compared with NDA group,heart rate,FPG,2hPG,HbAIc,TG,LDL-C and hs-CRP levels of DA group were signifieantly increased （ P 〈 0.05 ）. SAS seores were positively correlated with heart rate, FPG,2hPG, HbA 1 c, TG, LDL-C and hs-CRP levels. Multiple stepwise regression analysis showed that 2hPG HbAl c, LDI,-C and hs-CRP levels had a significant impact on anxiety mood of type 2 diabetes patients （ P 〈 0.05 ）. Conclusion Anxiety mood and glu- colipid metabolic disorders, chronie inflammation response were closely related in type 2 diabetes, especially, 2hPG, HbA1c, LDL-C and hs-CRP, may be involved in the occurrence of type 2 diabetes anxiety.
