摘  要:目的了解大肠埃希菌的耐药性,同时了解产ESBLs大肠埃希菌耐消毒剂基因qacE△1-sul1的流行情况,揭示大肠埃希菌的耐药机制。方法 2007年1月-2011年11月医院分离的3688株大肠埃希菌用VITEK-32GNI+鉴定,GNS-448检测对氨基糖苷类、β-内酰胺类、喹诺酮类等15种抗菌药物的耐药性,并对其中36株产ESBLs大肠埃希菌用PCR法检测耐消毒剂基因qacE△1-sul1的流行状况。结果 3688株大肠埃希菌中共检出1660株产ESBLs菌株,检出率为45.0%;大肠埃希菌除对阿米卡星、亚胺培南、美罗培南、头孢西丁、头孢哌酮/舒巴坦、哌拉西林/他唑巴坦耐药率较低外,对其他抗菌药物均产生了一定的耐药性,36株产ESBLs大肠埃希菌对氨苄西林、头孢唑林、头孢呋辛、庆大霉素全耐药,未检出亚胺培南、美罗培南耐药株;36株产ESBLs大肠埃希菌耐消毒剂基因qacE△1-sul1检测结果22株阳性,检出率为61.1%。结论大肠埃希菌耐药性严重,但近几年基本稳定,产ESBLs大肠埃希菌存在多药耐药性;产ESBLs大肠埃希菌耐消毒剂基因qacE△1-sul1检出率较高,且与多药耐药关系密切。OBJECTIVE To investigate the drug resistance of Escherichia coli (E. coli), the prevalence of the disinfectant-resistant genes qacE△l-sull in ESBLs-producing E. coli so as to announce the drug resistance mechanism of E. coli. METHODS A total of 3688 strains of E. coli were analyzed for the drug resistance to 15 kinds of antibiotics such as aminoglycoside, β-1actams and quinolones by using VITEK-32 GNS-448 cards following GNI+ cards for the identification, which were isolated for the hospital from Jan 2007 to Nov 2011. PCR method was used to analyze qacEfl-sullgenes in 36 strains of ESBLs-producing E. coli so as to study their prevalence. RESULTS Totally 1660 strains of ESBLs-producing E. coli were found with a positive rate of 45.0%. The drug resistance rates of the E. coli strains to amikacin, imipenem, meropenem, cefoxitin, levofloxacin, cefoperazone/sulbactam and piperacillin/tazobactam were relatively low, and the strains had produced certain drug resistance to other antibiotics. The result of the drug susceptibility testing of the 36 strains of ESBLs-producing E. coli indicated that the strains were fully resistant to ampicillin, cefazolin, cefuroxime, and gentamicin, and that no strains of imipenem-resistant or meropenem-resistant strains were detected; totally 22 strains were found positive for qacEfl-sull genes among the 36 strains of ESBLs-producing E. coli, with a positive rate of 61.1%. CONCLUSION The drug resistance of E. coli is severe, which is basically stable in the past several years. Furthermore, the ESBLs-producing E. coli strains show multi-drug resistance. The positive rate of the qacE△1- sull genes is high in the ESBLs-producing E. coli, which is closely related to its multi-drug resistance.
