幽门螺杆菌感染与胃癌中信号通路中效应蛋白及反式激活活性蛋白表达的关系

摘　　要：目的探讨幽门螺杆菌(Hp)感染与胃癌中信号通路中效应蛋白(Shh)和反式激活活性蛋白(C-myc)表达的关系,可作为胃上皮组织发生肿瘤化的重要指示基因。方法采用免疫组化法对100例胃癌组织及46例正常胃上皮组织中的Shh及C-myc进行检测,然后采用快速尿素酶试验及组织病理学检测两种临床检测方法对Hp进行检测,以了解Shh与C-myc在胃癌组织中的表达及其与Hp感染之间的关系。结果胃癌组织中Shh的平均染色得分为(4.02±2.31)分,明显高于正常胃上皮的染色总得分(1.19±0.87)分(P<0.05);胃癌组织中C-myc的平均染色得分为(4.59±2.58)分,明显高于正常胃上皮的染色总得分(2.11±1.77)分(P<0.05);Hp感染呈阳性与阴性胃癌中,Shh表达的阳性率分别为23.68%与69.35%,差异有统计学意义(P<0.05);在Hp感染呈阳性与阴性胃癌中,C-myc表达的阳性率分别为54.76%、74.14%,差异无统计学意义。结论胃癌组织中的Shh及C-myc两种基因的表达与Hp感染存在着一定的关系,可作为胃上皮组织发生肿瘤化的重要指示基因。OBJECTIVE To investigate the relationship between Helicobacter pylori （Hp） infection and the expression of gastric Shh and C-myc which can be served as an important indicator gene for the tumorigenesis of gastric epithelial tissue. METHODS The clinical expression of Shh and C-myc in 100 cases of gastric carcinoma and 46 cases of normal gastric epithelial tissues were detected by using immunohistochemical methods. Then the rapid detection urease test and histopathological detection were used to detect the Hp so as to understand the expression of Shh and C-myc in gastric carcinoma tissues and its correlation with Hp infection. RESULTS The average Shh staining score of the gastric cancer tissues was（4.02~d-2.31） points, significantly higher than （1.19-＋-0.87） points of the normal gastric epithelial tissues （P〈0.05） ; the average C-myc staining score of gastric carcinoma tissues was （4. 59±2.58） points, significantly higher than （2. 11 ± 1.77） points of the normal gastric epithelial tissues （P〈0.05）. The positive rate of the expression of Shh was 23. 68% in the Hp-positive gastric cancer patients, 69.35 % in the H p-negative gastric cancer patients, the difference was statistically significant（P〈0.05）; while the positive rate of the expression of C-myc was 54.76% in the Hp-positive gastric cancer patients, 74.14% in the Hp-negative gastric cancer patients, the difference was not statistically significant. CONCLUSION There is certain correlation between the expression of Shh and C-myc in gastric carcinoma tissues and the Hp infection, and they can serve as an important indicating gene for the tumorigenesis of gastric epithelial tissue.
