CT在肺部真菌感染中的临床诊断价值

摘　　要：目的分析肺部真菌感染CT影像学表现的特征,为该病的早期诊断提供理论依据。方法选择2009年1月-2012年6月在医院收治的肺部真菌感染患者39例,所有患者均行X线胸部正侧位检查和胸部CT平扫,观察患者肺内病灶的形态影像学特征,并比较分析其与病原学检查结果。结果肺部真菌感染患者中通过X线检查的阳性率为10.26%,CT检出率为74.36%,CT检出率明显高于X线检出率,两者比较差异有统计学意义(P<0.01);病原菌检查结果分别为肺隐球酵母菌病9例,占23.08%,肺曲霉菌病16例,占41.03%,假丝酵母菌病11例,占28.21%,胸部CT影像学表现为结节或肿块影14例占35.90%;晕征9例占23.08%,空洞13例占33.33%,其中洞内球征4例占10.26%,新月征7例占17.95%,渗出性病变8例占20.51%,肺气腔实变4例占10.26%。结论肺真菌感染表现多种多样,CT检查的特征性表现对提示和尽早治疗肺部真菌感染具有良好的临床参考价值。OBJECTIVE To analyze the CT imaging features of pulmonary fungal infections so as to provide theoretical basis for the early diagnosis of the disease. METHODS From Jan. 2009 to Jun. 2012,39 cases with pulmonary fungaI infections were enroIled in the study. AlI the patients were taken X-ray examination and CT scan. The morphological imaging features of the lung lesions were observed, and the results of the etiological examination were analyzed and compared. RESULTS The detection rate of X-ray was 10.26%, CT 74.36%, the detection rate of CT was significantly higher than that of X-ray, the difference was statistically significant （P〈 0.01）. The result of the etiological examination showed that there were 9 （23. 08%） patients with pulmonary eryptococcosis, 16 （41.03%） patients with pulmonary aspergillosis, and 11 （28. 21 %） patients with pulmonary candidiasis. The chest CT imaging manifestations indicated that there were 14 （35.90%）patients with nodule or mass,9 （23.08%） patients with hale signs and 13 （33. 33%） patients with cavity, among which there were 4 （10.26%） patients with the ball in side the cave,7 （17.95%） patients with air semilunar sign, 2 （20.51%） patients with exudative process, and 4 （10. 26%） patients with consolidation. CONCLUSION It is varied of the manifestations of the pulmonary fungal infections, the CT manifestation has good clinical reference value in the predication and early treatment of pulmonary fungal infections.
