超声心动图诊断亚急性细菌性心内膜炎赘生物的价值

摘　　要：目的探究并分析超声心动图在诊断亚急性细菌性心内膜炎(SBE)赘生物中的价值,为临床诊治提供有效的依据。方法选取1995年10月-2012年8月收治的SBE患者48例作为研究对象,48例患者术前均行超声心动图检查,将其诊断结果与手术诊断进行对比分析。结果超声心动图检查出的心内膜赘生物患者有46例,阳性率为95.83%,其中赘生物附着于主动脉瓣的患者有12例、二尖瓣9例、三尖瓣8例、肺动脉瓣10例、主动脉瓣合并二尖瓣7例;超声心动图检查出的赘生物大小在2～15mm的患者例数最多,占67.39%;赘生物数量为2个的患者例数最多,占56.52%;超声心动图诊断出的瓣膜赘生物附着部位及赘生物大小和数量与手术诊断结果相近,差异无统计学意义。结论超声心动图能够准确检查出赘生物的附着部位和赘生物的大小、数量,对感染性心内膜炎的诊断具有很大的意义,值得在临床上广泛推广。OBJECTIVE To explore and analyze the value of ultrasound heartbeat diagram in the diagnosis of subacute bacterial endocarditis （SBE） neoplasm so as to provide basis for the clinical diagnosis and treatment. METHODS A total of 48 SBE patients , who were treated in the hospital from Oct 1995 to Aug 2012, were selected as the research subjects. All the 48 patients underwent echocardiography examination, the results of the diagnosis were compared with the surgical diagnosis. RESULTS The ultrasonic cardiogram result showed that there were 46 cases with the endocardial vegetations with the positive rate of 95.83 %, among which there were 12 attached to the aortic valve vegetations, 9 cases of mitral valve, 8 cases of three tricuspid valve, 10 cases of pulmonary valve in 10 , and 7 cases of aortic and mitral valve; the ultrasonic cardiogram results showed that the vegetation size varying from 2 mm to 15 mm was the largest, accounting for 67.39%; the majority was the vegetation quantity of 2, occupying 56.52%. Ultrasound diagnosis of heartbeat of valvular vegetations attachment site and vegetation size and number were similar to the result of operation diagnosis , the difference was not statistically significant. CONCLUSION The ultrasound heartbeat graph can accurately check out the excrescence of the attachment site and vegetations size, and amount, it is of great significance in the diagnosis of infective endocarditis and is worthy of being widely promoted.
