温州民营医院感染管理存在的问题与对策

摘　　要：目的探讨民营医院的医院感染管理工作,提高民营医院医疗服务质量。方法依据浙江省基层医院感染管理检查统一评分标准,对温州市卫生局所辖的40所民营医院于2010-2012年进行了连续3年的质控检查并归纳分析总结。结果 35所民营医院得分在<90分,均不同程度地存在医院感染管理组织不健全、感染监测缺乏、科室管理不规范等问题,但将2012年的检查结果与历年相比,有明显提高,如供应室布局不合理率已由2010年的87.5%下降至2011年的70.0%,最终降至2012年的37.5%;灭菌器监测不规范率由2010年的45.0%降至2012年的25.0%;手术室布局流程不合理率由2010年的22.5%降至2012年的10.0%;无菌包包装不规范率由2010年的35.0%降至2012年的22.5%等。结论民营医院由于人员编制、管理与医疗质量基础的限制使其与国营医院有较大距离,但卫生行政部门通过加大监督管理处罚力度,并辅以不间断地质控检查和持续不断的整改,可获得明显管理成效。OBJECTIVE To make the hospital infection management of the private hospitals more standard so as to improve the quality of medical service. METHODS According to Zhejiang basic-level hospital infection control standard checklist, forty private hospitals under the jurisdiction of Wenzhou health bureau were accepted the quality inspections consecutively from 2010 to 2012 and then scores and problems were summarized and analyzed. RESULTS Thirty-five private hospitals got scores below 90 points. There were many problems such as the incomplete organizations for management, lack of infection surveillance and unstandardized management of departments. As compared with the inspection result of 2012, there were still some improvements to be made, such as the rate of unreasonable-layout of the supply department dropping from 87.5 % in 2010 to 70.0 % in 2012, the rate of unstandard surveillance of sterilizer decreasing from 45. 0% in 2010 to 25. 0% in 2012, the rate of unreasonable layout of the operating room dropping from 22. 5% in 2010 to 10. 0% in 2012, and the rate of unstandard package of aseptic package decreasing from 35.0% in 2010 to 22.5% in 2012. CONCLUSION There is still big gap between the private hospitals and the public hospitals due to the restriction of the staffing, management, and the foundation of medical quality, however, the significant management effectiveness can be achieved by the health administrative department＇s increasing the supervision and continuous inspection and rectification.
