头穴透刺配合康复训练治疗脑卒中后肩手综合征疗效观察

摘　　要：目的观察头穴透刺配合康复训练治疗脑卒中后肩手综合征的临床疗效。方法将80例脑卒中后肩手综合征患者随机分为治疗组和对照组,每组40例。治疗组采用头穴透刺配合康复治疗,对照组采用单纯康复治疗。观察两组治疗前后患肢疼痛程度(VAS评分)及肩手部位皮肤温度以及上肢功能评定(Carol)评分,比较两组临床疗效。结果治疗组总有效率为100.0%,对照组为95.0%,两组比较差异具有统计学意义(P<0.01)。两组治疗后VAS评分及患肢Carol评分与同组治疗前比较,差异均具有统计学意义(P<0.01)。治疗组治疗后VAS评分及患肢Caro1评分与对照组比较,差异均具有统计学意义(P<0.01)。两组治疗后患健侧肩、手皮肤温度差与同组治疗前比较,差异均具有统计学意义(P<0.01)。结论头穴透刺配合康复训练是一种治疗脑卒中后肩手综合征的有效方法。Objective To investigate the clinical efficacy of scalp point-to-point acupuncture plus rehabilitation training in treating post-stroke shoulder-hand syndrome. Methods Eighty patients with post-stroke shoulder-hand syndrome were randomly allocated to treatment and control groups, 40 cases each. The treatment group received scalp point-to-point acupuncture plus rehabilitation training and the control group, rehabilitation training alone. The severity of pains in the affected limb and upper limb function were assessed using the VAS and Carroll UEFT respectively and shoulder and hand skin temperature was measured in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups. Results The total efficacy rate was 100% in the treatment group and 95.0% in the control group; there was a statistically significant difference between the two groups （P〈0.01）. There were statistically significant pre-/post-treatment differences in the VAS score and the Carroll UEFT score in the affected limb in the two groups （P^0.01）. There were statistically significant post-treatment differences in the VAS score and the Carroll UEFT score in the affected limb between the treatment and control groups （P〈0.01）. There were statistically significant pre-/post-treatment differences in shoulder and hand skin temperature differences between the healthy and affected sides in the two groups （P（0.01）. Conclusion Scalp point-to-point acupuncture plus rehabilitation training is an effective way to treat post-stroke shoulder-hand syndrome.
