摘  要:目的:总结不同指动脉岛状皮瓣在修复手部皮肤软组织缺损中的临床应用及疗效。方法:采用不同指动脉岛状皮瓣修复87例手部皮肤软组织缺损,其中采用指动脉逆行岛状皮瓣27例,指动脉背侧支逆行岛状皮瓣12例,指动脉背侧支岛状皮瓣7例,指动脉顺行岛状皮瓣10例,邻指指动脉顺行串联皮瓣8例,邻指侧方皮瓣联合掌背动脉逆行岛状皮瓣23例。结果:术后皮瓣全部存活,2例采用指动脉逆行岛状皮瓣和1例采用指动脉背侧支逆行岛状皮瓣修复者远端部分坏死,经换药后愈合。术后随访6~13个月,皮瓣外形、质地良好,伤手功能恢复满意。结论:指动脉系列岛状皮瓣是修复手部皮肤软组织缺损的较好方法,根据手部皮肤软组织缺损情况选择不同术式的皮瓣修复可获得理想的修复效果。Objective: To summarize the selection and clinical results of different types of digital arterial island flap for repair of soft tissue defect of the hand. Methods: Eighty- seven cases of soft tissue defect of the hand were treated with 6 kinds of digital arterial island flap: the digital arterial reverse island flap in 27 case, the reverse island flap of dorsal branches of the digital arterial in 12 case, the island flap of the digital arterial dorsal branches in 7 case, the island flaps pedicled with digital artery in 10 case, the tandem skin flap of digital arterial in 8 case, the neighborly finger lateral island flap combined with the reverse dorsal metacarpal artery island flap in 27 case.Results: ALL case survived completely, 2 case treated with the digital arterial reverse island flap and 1 case treated with the reverse island flap of dorsal branches of the digital arterial were necrosised and healed after change dressings. Postoperative follow-up period was 6 to 13 months. All these flaps recovered with satisfying appearance and quality. The function of injured hand was good after reconstruction. Conclusion: According to locations and characteristics of the hand injury, choosing the most appropriate kind of series island flaps derived from digital arterial is the optimal way for the treatment of soft tissue defect of the hand, and ideal result can be obtained.
