次全椎板减压、单枚cage后路椎体间融合术治疗腰椎不稳症临床疗效

摘　　要：目的:评价次全椎板切除减压、斜向植入单枚椎体融合器(cage)后路椎体间融合术治疗腰椎不稳症的临床疗效。方法:对26例采用次全椎板减压、单枚cage联合椎弓根螺钉治疗的腰椎不稳症患者的临床资料进行回顾性分析。26例均为单节段病变,术后定期随访,比较术前、术后相对椎间隙高度(椎间隙前、中、后部高度之和与下椎体前后径的比值)。按SUK标准观察椎体间融合情况,采用JOA评分及ODI评分进行临床疗效的评估。结果:相对椎间隙高度术后与术前比较差异有统计学意义(P<0.05);术后半年随访时椎体间融合率为92.31%。JOA评分优17例、良7例、中2例,优良率为92.31%;半年随访ODI评分由术前(52.61±6.75)分降至(6.42±2.39)分。结论:次全椎板减压、单枚cage后路椎体间融合术是一种治疗腰椎不稳症的有效手术方法。Objective： To evaluate the clinical curative effect of the posterior lumbar interbody fusion with subtotal laminectomy decompression and single cage insertion to treat unstable lumbar vertebrae. Methods： Twenty-six cases of lumbar instability who had been treated with the lumbar interbody fusion with single cage were retrospectively studied. All cases who had single intervertenral space involved, had a regular follow-up after operation to compare relative vertebral gap height between preoperative and postoperative. Intervertebral fusion after operations was observed according to the standard of SUK. JOA score system and ODI score system were used to evaluate the clinical effects. Results： Compared with preoperative, postoperative vertebral relative vertebral gap height between showed remarkable difference（P 〈 0.05）. The fusion ratio was 92% 6 months after operation. JOA score was excellent in 17 cases, good in 7 cases, fair in 2 cases, excellent and good rate was 92.31%.Preoperative ODI score was 52.61 ± 6. 75 and at final follow-up was 6.42 ± 2.39. Conclusion： The posterior lumbar interbody fusion with subtotal laminectomy decompression and single cage insertion are an effective technique in treating lumbar instability vertebrae.
