215份儿科病历调查及抗菌药物应用分析

摘　　要：目的:调查我院儿科抗菌药物使用情况,为临床合理使用抗菌药物提供参考。方法:调取我院2011年12月份儿内科全部出院归档病历共215份进行统计分析,内容包括患儿年龄、性别、入(出)院诊断、住院时间、抗菌药物应用情况等。结果:(1)儿科疾病以呼吸系统疾病和消化系统疾病为主;(2)199例使用了抗菌药物,使用率92.56%(199/215),涉及7类18种药物;(3)抗菌药物单用比例占68.34%;(4)非限制使用级、限制使用级、特殊使用级抗菌药物使用率分别为50.75%、80.40%、2.51%;(5)抗菌药物DDDs为47.95;(6)微生物检验样本送检率74.87%,其中有致病菌生长标本50份(26.74%)。结论:儿科微生物检验样本送检率和抗菌药物使用率较高,抗菌药物选择应侧重于非限制使用级,更换或联用应以微生物检验结果和近期全院细菌药物敏感趋势为主要依据,避免盲目、过度使用抗菌药物。Objective： To Provide a reasonable reference for clinical antimicrobial choice through investigating the use of antibacterial agents in internal medicine of pediatrics of our hospital. Methods： A total of 215 pediatric medical records in December 2011 were statistically analyzed, including children age, sex, admission diagnosis, discharge diagnosis, length of hospital stay, antimicrobial application, etc. Results： （1） Respiratory and digestive tract diseases were the main diseases in pediatrics department. （2） 199 children used the antibacterial drugs and the utilization rate was 93.46% , which involved 18 drugs in seven kinds. （3） The use of one kind of antimicrobial drugs accounted for 68.34%. （4） The utilization rate of ＂unrestricted＂, ＂restricted＂ and ＂special＂ antimicrobial drug were 50.75% , 80.40% and 2.51% , respectively. （5） DDDs was 47.95. （6） The submission rate of microbiological testing samples was 74.87%. Among them, there were 50 samples （26.74%） containing pathogenic bacteria growth. Conclusions： The rate of tested microbial samples and the utilization rate of antimicrobial drug are relatively high. Medication use should be focused on the ＂unrestricted＂ antibacterial drugs. Replacing or couple-using of antimicrobial drug should base on microbial inspection results and the sensitive trend of antibacterial drugs to avoid abuse of antimicrobial drugs.
