49例老年全肺切除术后的监护与治疗

摘　　要：目的总结60岁以上老年患者一侧全肺切除术后的监护与治疗经验。方法分析我院49例60岁以上老年患者一侧全肺切除术后的监护与治疗措施。结果本组术后使用呼吸机18例,术后出现肺部感染10例,急性呼吸衰竭呼吸功能不全5例,心律失常21例,急性肺水肿3例,急性肾功能衰竭2例,应激性溃疡上消化道出血1例,经积极治疗后康复48例,死亡1例。结论严密地监护措施,合理的术后治疗计划,积极防治并发症,是确保老年全肺切除患者安全度过围手术期的关键。Objective To explore the intensive custody and treatment of patients aged 60 and over afer pneumonectomy. Meth- ods From 2006 to 2012, the intensive custody and treatment of 49 patients aged 60 and over afer pneumonectomy were retrospectively re- viewed and analyzed. Results 18 patients were underwent mechanical ventilation. Postoperative pulmonary infection in 10 ,acute respira- tory failure in 5, arrhythmias occurred in 21 cases, acute pulmonary edema in 3, acute renal failure in 2, stress ulcer upper gastrointestinal bleeding in 1. Of the 49 cases, 1 was died. Conclusion The intensive custody and reasonable treatment plan to prevent and treat com- plications afer pneumonectomy are essential to ensure the elderly patients safely.
